利拉鲁肽致心率增快2例分析

摘　　要：利拉鲁肽是一种新型降糖药,可降低致死性或非致死性心肌梗死及肾脏复合终点的发生率,心率增快是其常见的不良反应。本文报道使用利拉鲁肽注射液降糖治疗的2例糖尿病患者,病例1平时心率波动在70~75次/min,使用利拉鲁肽1.2mgqd,3d后患者心率为92次/min,增加琥珀酸美托洛尔缓释片剂量,患者心率降至75次/min,后停用利拉鲁肽。病例2急性非ST段抬高型心肌梗死后,心率控制在67~79次/min,使用利拉鲁肽注射液0.6mg qd,心率76次/min,剂量增至1.2mgqd,5d后心率波动在72~90次/min,剂量降至0.6mgqd,心率恢复至76次/min,药物调整期间增加琥珀酸美托洛尔缓释剂量,心率未明显降低。2例患者在使用利拉鲁肽1.2 mg qd 3~5 d后,心率均呈现增快现象。病例1增加琥珀酸美托洛尔缓释片剂量后,心率降低。病例2增加琥珀酸美托洛尔缓释片剂量后,心率降低并不明显,利拉鲁肽减量后,心率降低。建议临床在使用利拉鲁肽时注意监测患者心率,尤其是严重心力衰竭患者,以防严重不良反应的发生。Liraglutide is a novel hypoglycemic agent,which can reduce the incidence of fatal or non-fatal myocardial in-farction and renal complex endpoint,and rapid heart rate is a common adverse reaction.In this paper,two diabetic patients were treated with Liraglutide Injection for hypoglycemic treatment.In case 1,the normal heart rate fluctuated between 70 and 75 beats/min.After Liraglutide 1.2 mg qd was used,the patient's heart rate was 92 beats/min after 3 days,and the dosage of Metoprolol Succinate Sustained-release Tablets was increased,and the patient's heart rate was reduced to 75 beats/min,and Liraglutide was discontinued.In case 2,after acute non-ST elevation myocardial infarction,the heart rate was controlled at 67-79 beats/min,Liraglutide Injection was used at 0.6 mg qd,the heart rate was 76 beats/min,and the dose was increased to 1.2 mg qd,5 days later,the heart rate fluctuated at 72-90 beats/min,and the dose was reduced to 0.6 mg qd.The heart rate recovered to 76 beats/min,and the dosage of Metoprolol Succinate Sustained-release Tablets was in-creased during drug adjustment,and the heart rate did not decrease significantly.After 3 to 5 days of Liraglutide 1.2 mg qd,2 patients showed increased heart rate.Case 1 heart rate decreased after increasing the dosage of Metoprolol Succinate Sustained-release Tablets.Case 2 increased dosage of Metoprolol Succinate Sustained-release Tablets did not significantly reduce heart rate,but decreased heart rate after reduced dosage of Liraglutide.It is recommended to monitor patients'heart rate when using Liraglutide,especially in patients with severe heart failure,to prevent the occurrence of serious adverse reactions.
