骶四针穴针刺联合加味八珍汤治疗压力性尿失禁临床研究

摘　　要：目的:观察骶四针穴针刺联合加味八珍汤治疗压力性尿失禁的临床疗效。方法:选取2023年7月至2024年9月在濮阳市中医院治疗的压力性尿失禁患者126例,按照随机数字表法分为中医组和对照组,每组各63例。对照组给予常规治疗,中医组给予骶四针穴针刺联合加味八珍汤治疗。观察两组患者治疗前后中医证候积分、国际尿失禁咨询委员会尿失禁问卷简表(international consultation on incontinence modular questionnairestress-urinary incontinence short form,ICI-Q-SF)评分、尿动力学指标[最大尿流速(the maximum flow rate,Qmax)、残余尿量(Postvoid residual volume,PRV)、最大逼尿肌压(maximum detrusor pressure,Pdet.max)及最大膀胱容量(Maximum Cystometric Capacity,MCC)]、1 h尿垫试验变化情况及临床疗效。结果:中医组有效率为92.06%,对照组有效率为77.78%,中医组有效率高于对照组,差异有统计学意义(P<0.05);中医组治疗后中医证候积分均低于对照组,差异有统计学意义(P<0.05);中医组治疗后ICI-Q-SF评分为(4.52±0.55)分,低于对照组的(6.15±0.78)分,差异有统计学意义(P<0.05);中医组治疗后Qmax、Pdet和MCC水平高于对照组,中医组治疗后PRV水平低于对照组,差异有统计学意义(P<0.05);中医组治疗后1 h尿垫试验尿垫重量低于对照组,差异有统计学意义(P<0.05)。结论:骶四针穴针刺联合加味八珍汤治疗压力性尿失禁能够有效改善患者尿失禁症状,提高临床疗效。Objective:To observe the clinical efficacy of Sacral Four-Needle Acupuncture combined with Modified Eight-Gem Decoction on stress urinary incontinence(SUI).Methods:A total of 126 SUI patients treated at Puyang Hospital of Traditional Chinese Medicine from July 2023 to September 2024 were selected and divided into the TCM group and the control group using the random number table method,with 63 cases in each group.The control group received conventional treatment,while the TCM group received Sacral Four-Needle Acupuncture combined with Modified Eight-Gem Decoction.The changes in TCM syndrome scores,international consultation on incontinence modular questionnaire-stress urinary incontinence short form(ICI-Q-SF)scores,urodynamic indicators[such as maximum flow rate(Qmax),postvoid residual volume(PRV),maximum detrusor pressure(Pdet.max),maximum cystometric capacity(MCC)],1-hour pad test results before and after treatment,and clinical efficacy were observed.Results:The effective rate of the TCM group was 92.06%,which was higher than that of the control group:77.78%,and the difference was statistically significant(P=0.025<0.05).After treatment,the TCM syndrome scores of the TCM group were lower than those of the control group,and the difference was statistically significant(P<0.05).The ICI-Q-SF score of the TCM group was(4.52±0.55)points,which was lower than that of the control group(6.15±0.78)points,and the difference was statistically significant(P<0.05).The levels of Qmax,Pdet and MCC in the TCM group were higher than those in the control group,and the level of PRV in the TCM group was lower than that in the control group,and the differences were statistically significant(P<0.05).The weight of the urine pad in the 1-hour urine pad test of the TCM group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:Sacral Four-Needle acupuncture combined with Modified Eight-Gem Decoction can effectively improve urinary incontinence symptoms and enhance clinical efficacy
