腹部拨筋结合八卦推拿对结直肠腺瘤患者便秘及炎症指标的影响

摘　　要：目的:观察腹部拨筋结合八卦推拿对结直肠腺瘤患者便秘及炎症指标的影响。方法:选取2024年2—7月在江苏省中医院消化内镜病房住院治疗的结直肠腺瘤患者70例,按照随机数字表法分为对照组和观察组,每组各35例。对照组给予常规护理、饮食指导,指导患者口服乳果糖,观察组在对照组治疗的基础上行腹部拨筋联合八卦推拿治疗。观察两组患者初次排便时间、间隔排便时间、72 h排便次数、白细胞介素(interleukin,IL)-6、肿瘤坏死因子(tumor necrosis factor,TNF)-α、C反应蛋白(C reactive protein,CRP)、炎症恢复情况等。结果:观察组治愈率为54.29%,对照组治愈率为11.43%,观察组治愈率高于对照组,差异有统计学意义(P<0.05);观察组初次排便时间、间隔排便时间、72 h排便次数优于对照组,差异有统计学意义(P<0.05);观察组炎症指标恢复正常率为71.42%,对照组炎症指标恢复正常率为25.71%,观察组高于对照组,差异有统计学意义(P<0.05);观察组治疗后CRP、TNF-α、IL-6低于对照组,差异有统计学意义(P<0.05)。结论:腹部拨筋结合八卦推拿可以改善结直肠腺瘤患者的便秘症状,降低炎症指标,提高患者生活质量。Objective:To observe the effects of abdominal tendon manipulation combined with Bagua massage on constipation and inflammatory indicators in patients with colorectal adenoma(CRA).Methods:A total of 70 patients with CRA hospitalized in the Endoscopy Ward of Jiangsu Province Hospital of Chinese Medicine from February 2024 to July 2024 were selected and randomly divided into the control group and the observation group using the random number table method,with 35 cases in each group.The control group received routine nursing care,dietary guidance,and oral lactulose administration,while the observation group received additional abdominal tendon manipulation combined with Bagua massage based on the control group′s treatment.Outcomes including first defecation time,defecation interval time,72-hour defecation frequency,interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),C-reactive protein(CRP),and inflammatory recovery,etc.were observed.Results:The cure rate in the observation group was 54.29%,which was higher than 11.43%in the control group,and the difference was statistically significant(P<0.05).The observation group showed superior outcomes in the first defecation time,defecation interval time,and 72-hour defecation frequency compared to those in the control group,and all the differences were statistically significant(P<0.05).The inflammatory indicator normalization rate in the observation group was 71.42%,which was significantly higher than that in the control group:25.71%,and the difference was statistically significant(P<0.05).After treatment,CRP,TNF-α,and IL-6 levels in the observation group were significantly lower than those in the control group,and all the differences were statistically significant(P<0.05).Conclusion:Abdominal tendon manipulation combined with Bagua massage can alleviate constipation symptoms,reduce inflammatory indicators,and improve quality of life in patients with CRA.
