卡前列甲酯联合缩宫素治疗高危妊娠阴道分娩产后出血患者的效果观察

摘　　要：目的观察卡前列甲酯联合缩宫素治疗高危妊娠阴道分娩产后出血(PPH)患者的效果。方法选取2022年1月—2023年12月金乡县人民医院收治的高危妊娠阴道分娩PPH患者60例,根据随机数字表法分为两组各30例,对照组采用缩宫素治疗,观察组采用卡前列甲酯联合缩宫素治疗。比较两组临床疗效、出血量、血流动力学指标[心率(HR)、平均动脉压(MAP)、血氧饱和度(SpO_(2))]以及不良反应发生情况。结果观察组总有效率高于对照组,观察组产后2、24 h出血量少于对照组(P<0.05);给药后10、30 min,两组HR、MAP、SpO_(2)水平降低,且观察组低于对照组(P<0.05);两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论卡前列甲酯联合缩宫素治疗高危妊娠阴道分娩PPH具有良好的止血效果,能有效改善患者血流动力学指标,且不良反应少。Objective To observe the effect of Carboprost tromethamine combined with Oxytocin in the treatment of postpartum hemorrhage(PPH)in high-risk pregnancy vaginal delivery patients.Methods A total of 60 PPH in high-risk pregnancy vaginal delivery patients admitted to Jinxiang People's Hospital from January 2022 to December 2023 were selected and randomly divided into two groups,30 cases in each group,the control group was treated with Oxytocin alone,while the observation group was treated with Carboprost tromethamine combined with Oxytocin.The clinical efficacy,postpartum blood loss,changes in hemodynamic indicators[heart rate(HR),mean arterial pressure(MAP),and pulse oxygen saturation(SpO_(2))],and the incidence of adverse reactions were compared between the two groups.Results The total effective rate in the observation group was higher than that in the control group,the blood loss at 2 and 24 hours after delivery in the observation group was less than that in the control group(P<0.05);at 10 min and 30 min after administration,the levels of HR,MAP,and SpO_(2) decreased in both groups,and the levels in the observation group were lower than those in the control group(P<0.05);there was no statistically significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusions Carboprost tromethamine combined with Oxytocin has a good hemostatic effect in the treatment of PPH in high-risk pregnancy vaginal delivery,effectively improving hemodynamic indicators with fewer adverse reactions.
