基于分层管理理论的护理干预模式在老年高血压患者中的应用效果

摘　　要：目的分析基于分层管理理论的护理干预模式在老年高血压患者中的应用效果。方法选取2023年2月—2024年6月新泰市泉沟镇卫生院收治的98例老年高血压患者,根据随机数字表法分为观察组(49例)和对照组(49例)。对照组给予常规护理干预模式,观察组给予基于分层管理理论的护理干预模式。比较两组血压水平、生活质量、用药依从性、负性情绪、自我护理能力。结果干预后,两组收缩压(SBP)、舒张压(DBP)水平及焦虑自评量表(SAS)、抑郁自评量表(SDS)评分均低于干预前,世界卫生组织生活质量量表(WHOQOL-100)各维度评分及自我护理能力测定量表(ESCA)评分均高于干预前,且观察组SBP、DBP水平及SAS、SDS评分低于对照组,WHOQOL-100各维度评分及ESCA评分高于对照组(P<0.05);观察组用药总依从性高于对照组(P<0.05)。结论基于分层管理理论的护理干预模式能够有效控制老年高血压患者的血压水平,缓解焦虑、抑郁情绪,提升用药依从性,提高其自我护理能力和生活质量。Objective To analyze the application effect of nursing intervention mode based on hierarchical management theory in elderly patients with hypertension.Methods A total of 98 elderly patients with hypertension from February 2023 to June 2024 treated in Quangou Town Health Center,Xintai City were selected,and by random number table method,they were divided into observation group(49 cases)and control group(49 cases).The control group was given the routine nursing intervention mode,and the observation group was given the nursing intervention mode based on hierarchical management theory.The blood pressure level,quality of life,medication compliance,negative emotion and self-care ability were compared between the two groups.Results After intervention,the systolic blood pressure(SBP),diastolic blood pressure(DBP)levels,Self-Rating Anxiety Scale(SAS)and Self-Rating Depression Scale(SDS)scores of the two groups were lower than before intervention,the World Health Organization Quality of Life Scale(WHOQOL-100)scores and Self-Care Ability Scale(ESCA)scores were higher than before intervention,and in comparison of the control group,the SBP and DBP scores and SAS and SDS scores in the observation group were lower,the WHOQOL-100 scores and ESCA scores were higher(P<0.05),the medication compliance of the observation group was higher(P<0.05).Conclusions The nursing intervention mode based on hierarchical management theory can effectively control the blood pressure level of elderly patients with hypertension,relieve anxiety and depression,improve medication compliance,and improve their self-care ability and quality of life.
