摘　　要：本文报道了1例婴儿外周静脉留置针拔管困难的护理经验。系统探讨了拔管困难可能的原因(如静脉管腔细小、静脉血栓形成和血管痉挛现象)、采取的护理措施,包括心理护理、早期识别与汇报、评估留置时长、局部热敷疗法、轻柔按摩、逐步尝试拔管、应急预案以及拔管后的护理措施,提出了相应的预防措施,并针对该患儿复诊情况建立了预警机制。最终,通过一系列护理干预措施,成功解决了拔管困难的问题,确保了患儿静脉输液治疗的安全性和有效性,为临床护士解决类似问题提供了参考依据。This article reports the nursing experience of an infant with difficulty in extubation of peripheral venous indentation needle.The possible causes of extubation difficulties(such as small venous cavity,venous thrombosis and vasospasm),nursing measures taken,including psychological care,early identification and reporting,evaluation of retention time,local hot compress therapy,gentle massage,gradual attempt to extubation,emergency plan and post-extubation nursing measures were systematically discussed,and corresponding preventive measures were proposed,an early warning mechanism was established according to the rediagnosis of the child.Finally,through a series of nursing interventions,the problem of difficult extubation was successfully solved,ensuring the safety and effectiveness of intravenous infusion therapy for children,and providing a reference for clinical nurses to solve similar problems.
