摘　　要：目的探讨APN与T2DM伴肥胖的关系。方法将147例初诊T2DM患者分为正常体重组(18.5 kg/m^(2)≤BMI<24 kg/m^(2))89例,肥胖组(BMI≥28 kg/m^(2))58例,比较2组患者脂联素、性别、年龄、血糖指标(空腹血糖、OGTT-2h、HbA1c)、HOMR-IR、血脂指标(TC、TG、HDL-C、LDL-C)、吸烟、血压、饮酒、维生素D等的差异;并进行Logistic回归明确T2DM伴肥胖的独立危险因素。结果2组患者比较,OGTT-2h、年龄、HBA1c、性别、饮酒、HDLC、UA差异有统计学意义(P<0.05),APN差异无统计学意义(P>0.05)。Logistic回归分析显示:OGTT-2h、年龄、HBA1c进入方程,APN未进入方程。结论初诊T2DM存在低脂联素水平及明显的胰岛素抵抗,且这种状态与是否合并肥胖无关。年龄是T2DM伴肥胖的独立危险因素。Objective To investigate the relationship between adiponectin(APN)and obese patients with type 2 diabetes mellitus(T2DM).Methods A total of 147 newly diagnosed T2DM patients were divided into normal weight group(18.5 kg/m^(2)≤BMI<24 kg/m^(2))of 89 cases and an obesity group(BMI≥28 kg/m^(2))of 58 cases.The differences of APN,gender,age,blood glucose indicators(fasting blood glucose OGTT-2h、HbA1c),HOMR-IR,Blood lipid indicators(TC,TG,HDLC,LDLC),smoking,blood pressure,drinking and vitamin D were compared between the two groups.Logistic regression was performed to identify independent risk factors for T2DM patients with obesity.Results There were statistically significant differences in OGTT-2h,age,HBA1c,gender,alcohol consumption,HDLC,and UA between the two groups of patients(P<0.05),while there was no statistically significant difference in APN(P>0.05).Logistic regression analysis showed that OGTT-2h,age,and HBA1c could entered the equation,while APN did not entered the equation.Conclusions Newly diagnosed T2DM has low levels of adiponectin and significant insulin resistance,which is not related to the presence of obesity.Age is an independent risk factor for T2DM with obesity.
