GDM孕妇血清中RBP-4表达与胰岛素抵抗及母婴结局的相关性

摘　　要：目的探讨GDM血清中RBP-4表达水平与胰岛素抵抗及不良妊娠结局的关系。方法分析126例GDM(GDM组)和40例正常妊娠(对照组)临床资料。GDM组分为不良妊娠结局组和妊娠结局良好组2种亚组。分析RBP-4表达水平与胰岛素抵抗及不良妊娠结局的关系。结果与对照组比,GDM组中RBP-4表达水平显著升高,差异有统计学意义(P<0.001)。与妊娠结局良好组比,不良妊娠结局组BMI、HOMA-IR和RBP-4表达水平显著升高,差异有统计学意义(P<0.05),HOMA-β显著降低,差异有统计学意义(P<0.05)。HOMA-IR和RBP-4是GDM发生不良妊娠结局的独立危险因素。GDM孕妇RBP-4与HOMA-IR具有显著正相关性(P=0.002)。RBP-4联合HOMA-IR在预测GDM发生不良妊娠结局临床效能显著(P<0.05)。结论GDM孕妇血清中RBP-4表达水平显著升高,RBP-4联合HOMA-IR在预测GDM发生不良妊娠结局中具有良好的价值。Objective To investigate the correlation between the serum expression level of RBP-4 and insulin resistance and adverse pregnancy outcomes in pregnant women with gestational diabetes mellitus(GDM).Methods The clinical data of 126 pregnant women with GDM(GDM group)and 40 normal pregnant women(control group)in our hospital were analyzed.GDM group was divided into two subgroups:poor pregnancy outcome group and good pregnancy outcome group.Analyzed the relationship between RBP-4 expression level and insulin resistance and adverse pregnancy outcomes.Results Compared with the control group,the serum expression level of RBP-4 in GDM group was significantly increased(P<0.001).Compared with the good pregnancy outcomes group,the expression level of BMI,HOMA-IR and RBP-4 were significantly increased in adverse pregnancy outcomes group(P<0.05),while HOMA-βwas significantly decreased(P<0.05).The HOMA-IR and RBP-4 were independent risk factors for adverse pregnancy outcomes in GDM pregnant women.There was a significant positive correlation between RBP-4 and HOMA-IR in pregnant women with GDM(P=0.002).RBP-4 combined with HOMA-IR had significant clinical efficacy in predicting adverse pregnancy outcomes in GDM pregnant women(P<0.05).Conclusions The serum expression level of RBP-4 in pregnant women with GDM was significantly increased,and the RBP-4 combined with HOMA-IR has a good clinical value in predicting the adverse pregnancy outcomes in pregnant women with GDM.
