摘 要:目的 分析重组人脑利钠肽(rh-BNP)对老年难治性心力衰竭(RHF)患者心肾功能以及炎症因子的影响。方法 选取我院2021年12月~2022年12月收治的108例老年RHF患者,随机分为对照组和rh-BNP组,两组均给予常规抗心衰治疗,rh-BNP组在常规抗心衰基础上加用rh-BNP。观察两组治疗前后临床症状、超声心动图指标、估算肾小球滤过率(eGFR)、血浆氨基末端脑钠肽前体(NT-proBNP)、高敏肌钙蛋白I(cTnI)及炎症因子白介素6(IL-6)、超敏C反应蛋白(hsCRP)的变化。对比两组患者不良反应发生情况及3个月内再住院率。根据数据资料的正态分布和方差齐性检验,两组之间采用t检验或U检验,以P<0.05为差异有统计学意义。结果 治疗后rh-BNP组总有效率显著高于对照组(P<0.05);rh-BNP组治疗后左室射血分数(LVEF)、每搏输出量(SV)较对照组高(P<0.05),左心室收缩末期内径(LVESD)及左心室舒张末期内径(LVEDD)均较对照组低(P<0.05);eGFR较对照组高(P<0.05);NT-proBNP、cTnI以及IL-6、hsCRP均较对照组低(P<0.05)。两组患者不良反应发生率及3个月内再住院率无差异(P>0.05)。结论 rh-BNP可显著改善老年RHF患者心肾功能,且不增加不良反应发生率,但不能降低老年RHF3个月内再住院率。Objective To explore the effects of recombinant human brain natriuretic peptide(rhBNP)on cardiac function,renal function,and inflammatory markers in elderly patients with RHF.Methods A total of 108 elderly patients diagnosed with RHF,who were admitted to our hospital between December 2021 and December 2022,were ran-domly allocated into a control group and a rhBNP group.Both groups received standard conventional anti-heart failure treatment.Additionally,the rhBNP group was administered rhBNP in addition to the conventional regimen.The study compared the two groups in terms of changes in clinical symptoms,echocardiographic parameters,estimated glomerular filtration rate(eGFR),plasma levels of N-terminal pro-brain natriuretic peptide(NT-proBNP),high-sensitivity troponin I(cTnⅠ),and inflammatory factors such as interleukin-6(IL-6)and high-sensitivity C-reactive protein(hsCRP).The inci-dence of adverse events and the 3-month readmission rate were also monitored in both groups.Based on the results of normal distribution and homogeneity of variance tests for the data,either the t-test or the U-test was employed for be-tween-group comparisons,with P<0.05 considered statistically significant.Results The overall treatment efficacy in the rhBNP group was significantly higher than that in the control group(P<0.05).Specifically,the left ventricular ejection fraction(LVEF)and stroke volume(SV)were significantly higher in the rhBNP group compared to the control group,while the left ventricular end-systolic diameter(LVESD)and left ventricular end-diastolic diameter(LVEDD)were sig-nificantly lower.The eGFR was also higher in the rhBNP group.Moreover,the levels of NT-proBNP,cTnⅠ,IL-6,and hsCRP were all significantly lower in the rhBNP group.All these differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse events or the 3-month readmission rate between the two groups(P>0.05).Conclusion The administration of rhBNP can significantly improve cardiovascular and rena
