CD38、CD138在子宫内膜息肉合并肥胖症患者中的表达差异性研究

摘　　要：目的 检测不同体重指数的子宫内膜息肉(EP)患者子宫内膜组织中CD38、CD138的表达情况,探究二者在肥胖与正常体重人群的表达差异性。方法 选取2021年7月~2023年12月于山东阳光融和医院妇科就诊的EP患者327例为研究对象,其中合并肥胖症患者74例(肥胖组),正常体重患者253例(正常组)。所有患者均行宫腔镜子宫内膜息肉电切术,收集两组患者一般资料,同时采用免疫组织化学染色法检测子宫内膜组织中CD38、CD138的表达情况,酶联免疫吸附法检测血清TNF-α、IL-6、CRP表达水平。结果 两组患者的年龄、月经周期、孕产史、息肉数目、最大息肉直径比较,差异均无统计学意义(P>0.05)。肥胖组子宫内膜组织中CD38、CD138阳性率分别为79.7%,74.3%,正常组患者分别为57.3%,62.1%,两组比较,差异有统计学意义(P<0.05),肥胖组患者血清中TNF-α、IL-6、CRP表达水平均高于正常组(P<0.05)。CD38、CD138与TNF-α、IL-6、CRP呈正相关(P均<0.05)。结论 在EP患者中,合并肥胖症患者的病灶组织中CD38、CD138表达较正常体重患者显著升高,可能与肥胖患者体内慢性炎症有关,对肥胖患者的EP治疗、长期管理以及后续生育计划的系统性诊疗提供指导作用。Objective To detect the expression of CD38 and CD138 in the endometrial tissues of endometri-al polyps(EP)patients with different body mass indexes,and to investigate the differences in the expression of CD38 and CD 138 in the obese and normal-weight populations.Methods Three hundred and twenty-seven cases of EP patients who attended the Department of Gynecology of Shandong Sunshine Union Hospital from July 2021 to December 2023 were se-lected for the study,among which 74 cases were combined with obese patients(obese group)and 253 cases were normal weight patients(normal group).All patients underwent hysteroscopic endometrial polypectomy,and the general data of the two groups were collected,while immunohistochemical staining was used to detect the expression of CD38 and CD138 in endometrial tissues,and enzyme-linked immunosorbent assay was used to detect the expression levels of serum TNF-al-pha,IL-6,and CRP.Results There was no statistically significant difference between the two groups of patients in terms of age,menstrual cycle,pregnancy and childbirth history,number of polyps,and diameter of the largest polyp(P>0.05).The positivity rate of CD38 and CD138 in endometrial tissues of obese group was 79.7%and 74.3%,respectively,and 57.3%and 62.1%in patients of normal group,which were significantly higher than that of normal group(P<0.05),and the expression levels of TNF-alpha,IL-6,and CRP in serum of patients of obese group were higher than that of normal group(P<0.05).Spearman's method analysis showed a positive correlation between CD38 and CD138 and TNF-α,IL-6,and CRP(all P<0.05).Conclusion In EP patients,the expressions of CD38 and CD138 in the focal tissues of pa-tients with combined obesity was significantly higher than those of patients with normal weight,which may be related to chronic inflammation in the body of obese patients,and provide guidance for the systematic diagnosis and treatment of EP treatment,long-term management and subsequent fertility planning in obese patients.
