右旋糖酐铁口服液对慢性心力衰竭合并铁缺乏患者心功能的影响

摘　　要：目的 探讨右旋糖酐铁口服液对慢性心力衰竭合并铁缺乏患者心功能的影响。方法 选取100例冠心病引起的慢性心力衰竭合并血清铁蛋白降低患者,分为治疗A组、对照组B组、治疗C组和对照D组。所有入选患者在入选时、治疗6个月后分别测量NT-proBNP、6 min步行距离,并根据临床表现确定其NYHA心功能分级。结果 同组治疗前后比较:A、B、C、D 4组患者治疗6个月后,NT-proBNP和NYHA心功能分级、6 min步行距离均明显好转(P<0.05);不同组间治疗后比较:A组患者与B组患者比较NT-proBNP下降(P<0.05),A组治疗后心功能分级较B组明显好转(P<0.05),A组治疗后6 min步行距离较B组明显提高(P<0.05)。C组患者较D组患者治疗后NT-proBNP、心功能分级、6 min步行距离均无统计学差异(P>0.05)。结论 对于慢性心力衰竭合并铁缺乏的缺铁性贫血患者,口服右旋糖酐铁口服液能改善患者的心功能;对于慢性心力衰竭仅仅合并铁缺乏,而没有缺铁性贫血患者,口服右旋糖酐铁口服液不能改善患者的心功能。Objective To explore the effects of adding iron dextran oral solution on cardiac function in pa-tients with chronic heart failure complicated with iron deficiency.Methods One hundred patients with chronic heart failure and decreased serum ferritin caused by coronary heart disease were included in this study.According to their he-moglobin values,they are divided into anemia group and non-anemia group.Patients in anemia group were divided into 2 groups:treatment group A and control group B.Patients in non-anemia group were divided into 2 groups:treatment group C and control group D.All selected patients were measured for NT proBNP and 6-minute walk test at the time of enroll-ment and after 6 months of treatment,and their NYHA heart function grading was determined based on clinical manifesta-tions.Results After 6 months of randomized treatment,patients in groups A,B,C and D showed significant improvement in NT proBNP,NYHA heart function grading,and 6-minute walk test compared to before treatment(P<0.05);Compari-son between different groups after treatment:The NT proBNP level in group A decreased compared to group B(P<0.05),the cardiac function grading in group A improved significantly compared to group B(P<0.05),and the 6-minute walk test in group A increased significantly compared to group B(P<0.05).There were no statistically significant differ-ences(P>0.05)in NT proBNP,cardiac function grading,and 6-minute walk test between group C and group D patients after treatment.Conclusion For patients with iron deficiency anemia complicated with chronic heart failure,oral admin-istration of iron dextran oral solution can improve their cardiac function;For patients with chronic heart failure who only have iron deficiency without iron deficiency anemia,oral administration of iron dextran oral solution cannot improve their cardiac function.
