摘 要:目的构建成年肿瘤患者上肢PICC导管尖端位置最佳置入长度的预测模型。方法选取2020年4月至2024年4月某医院行PICC导管置入的7331例成年肿瘤患者临床资料,单因素和多因素分析筛选出影响中心静脉腔内导管置入最佳长度的主要因素,基于影响因素构建预测模型。结果身高(OR=6.017)、体质指数(OR=2.443)、置入静脉(OR=94.780)、预测长度(OR=3.064)是成年肿瘤患者PICC导管尖端位置的影响因素(P<0.01),基于上述影响因素建立预测模型为Y=6.581+0.098×身高+0.015×体质指数-0.630×置入静脉+0.458×预测长度(R^(2)=0.232,F=441.255,P<0.001)。结论基于成年肿瘤患者腔内心电定位构建的模型,预测PICC导管的最佳置入长度、尖端定位具有较强的预示作用,为避免术中反复调整PICC导管提供了理论依据。Objective To construct a prediction model for the optimal placement length of peripherally inserted central venous catheters(PICC)in the upper limbs of adult cancer patients.Methods Clinical data of 7331 adult cancer patients who underwent PICC catheter placement in a hospital from April 2020 to April 2024 were selected.The main factors affecting the optimal length of central venous catheter insertion were screened out by univariate and multivariate analysis.The prediction model was constructed based on the influencing factors and the prediction effect of the model was verified.Results Height(OR=6.017),body mass index(OR=2.443),the inserted vein(OR=94.780),and predicted length(OR=3.064)were the influencing factors for the tip position of the PICC catheter in adult cancer patients(P<0.01).Based on the above influencing factors,the prediction model was established as Y=6.581+0.098×height+0.015×body mass index-0.630×the inserted vein+0.458×predicted length(R^(2)=0.232,F=441.255,P<0.001).Conclusion The model constructed based on intracavitary electrocardiographic localization in adult cancer patients demonstrates strong predictive capabilities for predicting the optimal insertion length and tip positioning of PICC catheters,providing a theoretical basis to avoid repeated intraoperative adjustments of the catheter during surgery.
