中国城镇老年人社区居家养老意愿及其影响因素研究

摘　　要：目的系统分析中国城镇老年人对社区居家养老模式的参与意愿及其影响因素。方法检索Cochrane Library、PubMed、Embase、Web of Science、中国生物医学文献数据库、中国知网、万方数据知识服务平台、维普中文科技期刊数据库有关中国城镇老年人对社区居家养老模式的参与意愿或影响因素的横断面研究,检索时限为2013年1月1日至2024年2月4日。由两名具有循证实践学习经验的研究者根据纳入和排除标准独立筛选文献、提取数据,并根据美国卫生保健质量和研究机构推荐的横断面研究质量评价标准对纳入文献进行质量评价,采用RevMan 5.3及SPSS 26.0软件进行数据分析。结果共纳入22篇文献,纳入样本量14063例,文献质量评价均为中等水平。分析结果显示,中国城镇老年人对社区居家养老模式的参与意愿率为26%。亚组分析显示,中部地区和近5年内的社区居家养老意愿率更高。影响因素分析结果显示,学历[OR=2.18,95%CI(1.64,2.89),P<0.001]、居住方式[OR=0.52,95%CI(0.33,0.84),P=0.007]、社区内有无养老服务[OR=2.57,95%CI(1.48,4.45),P<0.001]、是否接受过社区养老服务[OR=3.71,95%CI(2.67,5.15),P<0.001]、保姆[OR=0.26,95%CI(0.15,0.43),P<0.001]、养老方式认知[OR=2.52,95%CI(1.03,6.18),P=0.040]是中国城镇老年人社区居家养老意愿的影响因素。结论中国城镇老年人社区居家养老意愿率偏低。初中及以上学历、独居、社区内有养老服务、接受过社区养老服务、没有保姆照顾、了解养老方式的城镇老年人更愿意社区居家养老,提示未来应针对以上人群重点开展社区居家养老服务。Objective To systematically evaluate the willingness to participate in community-based home care models and associated influencing factors among the urban elderly in China.Methods Cochrane Library,Pubmed,Embase,Web of Science,CBM,China National Knowledge Infrastructure,Wanfang Database,and VIP Database were searched for cross-sectional studies about the participation willingness or the influencing factors of the urban elderly people's willingness to participate in the community-based home care model.The search period was from January 1,2013 to February 4,2024.Two researchers with evidence-based practical learning experience independently screened literature and extracted data according to inclusion and exclusion criteria.The literature quality was evaluated according to the cross-sectional study quality evaluation criteria recommended by the Agency for Healthcare Research and Quality(AHRQ).RevMan 5.3 and SPSS 26.0 software were used for data analysis.Results A total of 22 literature with a sample size of 14,063 were included,and the quality assessment of the literature in the study was all at a medium level.The results of analysis showed that the willingness rate of the urban elderly to participate in the community-based home care model was 26%.Subgroup analysis showed that the willingness rate of participating in community-based home care model in the central region and the past 5 years was higher.Education background[OR=2.18,95%CI(1.64,2.89),P<0.001],mode of living[OR=0.52,95%CI(0.33,0.84),P=0.007],whether there were elderly care services in the community[OR=2.57,95%CI(1.48,4.45),P<0.001],whether they have received community elderly care services[OR=3.71,95%CI(2.67,5.15),P<0.001],nanny[OR=0.26,95%CI(0.15,0.43),P<0.001],and cognition of elderly care modes[OR=2.52,95%CI(1.03,6.18),P=0.040]were the factors affecting the willingness of participating in community-based home care model among urban elderly in China.Conclusion The willingness rate of participating in community-based home care model among the elderly in
