摘  要:目的分析川崎病合并冠状动脉病变患儿血清多配体蛋白聚糖1(SDC-1)、25-羟维生素D3[25(OH)D3]表达水平与肠道菌群失调的关系。方法回顾性抽取2019年1月至2024年1月西安市第三医院收治的川崎病患儿104例,根据冠状动脉病变发生情况将其分为合并冠状动脉病变患儿组(合并组,25例)与未合并冠状动脉病变患儿组(未合并组,79例)。比较合并组与未合并组临床资料、血清SDC-1和25(OH)D3表达水平、肠道菌群。通过Pearson相关分析研究川崎病合并冠状动脉病变患儿血清SDC-1、25(OH)D3表达水平与肠道菌群失调的关系。结果两组性别、年龄、体质量、川崎病类型、居住地比较,差异未见统计学意义(P>0.05)。合并组血清SDC-1水平高于未合并组,25(OH)D3表达水平低于未合并组(P<0.05)。合并组乳酸菌、韦荣氏球菌属、梭状芽胞杆菌水平低于未合并组,拟杆菌、肠球菌、副杆菌水平高于未合并组(P<0.05)。Pearson分析结果显示,血清SDC-1与乳酸菌(r=-0.73)、韦荣氏球菌属(r=-0.56)、梭状芽胞杆菌(r=-0.81)呈负相关(P<0.05);25(OH)D3与乳酸菌(r=0.81)、韦荣氏球菌属(r=0.78)、梭状芽胞杆菌(r=0.66)呈正相关(P<0.05);血清SDC-1与拟杆菌(r=0.86)、肠球菌(r=0.68)、副杆菌(r=0.80)呈正相关(P<0.05);25(OH)D3与拟杆菌(r=-0.86)、肠球菌(r=-0.62)、副杆菌(r=-0.66)呈负相关(P<0.05)。结论川崎病合并冠状动脉病变患儿血清SDC-1水平升高,25(OH)D3水平降低,且与肠道菌群失调密切相关。ObjectiveTo analyze the correlations of expressions of serum syndecan-1(SDC-1)and 25-hydroxyvitamin D3,referred to as 25(OH)D3,with dysregulation of intestinal flora in children with Kawasaki disease complicated by coronary artery disease.MethodsA total of 104 children with Kawasaki disease admitted to Xi’an No.3 Hospital from January 2019 to January 2024 were selected for the retrospective study.And they were divided into the combined group(25 cases)and the non-combined group(79 cases)according to the occurrence of coronary artery lesions.The clinical data,expression levels of SDC-1 and 25(OH)D3,and intestinal flora were compared between the combined group and the non-combined group.The correlations of the expression levels of SDC-1 and 25(OH)D3 with dysregulation of intestinal flora in Kawasaki disease children with coronary artery disease were analyzed by Pearson correlation method.ResultsThere was no significant difference in gender,age,body mass,Kawasaki disease type,residence between the two groups(P>0.05).Compared with the non-combined group,the combined group had higher level of SDC-1 and lower level of 25(OH)D3(P<0.05).The levels of Lactobacillus,Veillonella and Clostridium in the combined group were lower than those in the non-combined group,while the levels of Bacteroides,Enterococcus and Paracelobacter were higher than those in the non-combined group(P<0.05).Pearson correlation analysis showed that SDC-1 was negatively correlated with Lactobacillus(r=-0.73),Veillonella(r=-0.56)and Clostridium(r=-0.81,P<0.05);25(OH)D3 was positively correlated with Lactobacillus(r=0.81),Veillonella(r=0.78)and Clostridium(r=0.66,P<0.05).SDC-1 was positively correlated with Bacteroides(r=0.86),Enterococcus(r=0.68)and Paracelobacter(r=0.808,P<0.05).25(OH)D3 was negatively correlated with Bacteroides(r=-0.86),Enterococcus(r=-0.62)and Paracelobacter(r=-0.66,P<0.05).ConclusionsLevels of SDC-1 increases,and 25(OH)D3 decreases in children with Kawasaki disease complicated by coronary artery disease,and they are closely relat
