摘 要:目的探讨宫颈超声弹性成像技术(SWE)对孕期妇女宫颈机能的评估及临床应用价值。方法前瞻性抽取2023年11月至2024年8月宝鸡市中心医院收治的200例宫颈机能不全孕妇为观察组,依据2∶1配对原则,抽取同期健康孕妇100名为对照组。比较两组二维超声参数(宫颈管长度、宫颈管开放宽度、宫颈管开放长度)及SWE参数(B点、D点的宫颈硬度及应变速度),分析SWE对孕期妇女宫颈机能的诊断价值,比较观察组不同妊娠结局者SWE参数(B点、D点的宫颈硬度及应变速度)并分析其相关性。结果观察组宫颈管长度、宫颈管开放长度小于对照组,宫颈管开放宽度大于对照组(P<0.05);观察组B点、D点宫颈硬度及应变速度均高于对照组(P均<0.05)。SWE参数诊断孕期宫颈机能不全患者的曲线下面积为0.926,约登指数为0.805,敏感度为94.50%,特异度为86.00%(P<0.05);妊娠结局不良者B点、D点宫颈硬度及应变速度均高于妊娠结局良好者(P<0.05)。B点、D点宫颈硬度及应变速度与宫颈管长度、开放长度呈负相关(P<0.05),与开放宽度、宫颈机能不全、妊娠结局呈正相关(P<0.05)。结论SWE参数水平可有效反映孕期妇女宫颈机能不全的发生情况,临床检测SWE参数水平可对预测患者妊娠结局提供一定的参考价值。ObjectiveTo investigate the evaluation and clinical application value of cervical ultrasound shear-wave elastography(SWE)for cervical function in pregnant women.MethodsA total of 200 pregnant women with cervical dysfunction admitted to Baoji Central Hospital from November 2023 to August 2024 were prospectively selected as the observation group.And 100 healthy pregnant women in the same period were selected as the control group according to the 2∶1 matching principle.Two-dimensional ultrasound parameters(cervical canal length,cervical canal opening width,cervical canal opening length)and SWE parameters(cervical hardness and strain velocity at points B and D)were compared between the two groups.The diagnostic value of SWE for cervical function in pregnant women was analyzed.The SWE parameters(cervical hardness and strain velocity at points B and D)of patients with different pregnancy outcomes in the observation group were compared,and the correlation was analyzed.ResultsThe cervical canal length and cervical canal open length in the observation group were smaller than those in the control group,while the cervical canal open width in the observation group was larger than that in the control group(P<0.05).The cervical hardness and strain velocity at points B and D in the observation group were higher than those in the control group(all P<0.05).The area under the curve for SWE parameter in the diagnosis of cervical dysfunction patients during pregnancy was 0.926,the Youden index was 0.805,the sensitivity was 94.50%,and the specificity was 86.00%(P<0.05).Patients with poor pregnancy outcomes had higher cervical hardness and strain velocity at points B and D compared with patients with good pregnancy outcomes(P<0.05).At points B and D,cervical hardness and strain velocity were negatively correlated with cervical canal length and cervical canal open length(P<0.05),while they were positively correlated with cervical canal open width,cervical incompetence,and pregnancy outcome(P<0.05).ConclusionsThe level of SWE paramete
