摘 要:目的研究神经肌电图(EMG)用于腰椎间盘突出症(LDH)定位诊断的意义。方法回顾性抽取2021年1月至2023年12月商丘市第一人民医院收治的LDH患者78例,均进行EMG和磁共振成像(MRI)检查,统计EMG与MRI对LDH受累神经节段的定位情况,并比较健侧与患侧EMG神经传导指标。结果EMG对LDH受累神经节段第2~3腰椎的检出率(1.28%,1/78)、第4腰椎的检出率(25.64%,20/78)、第5腰椎的检出率(94.87%,74/78)、第1骶椎的检出率(84.62%,66/78)与MRI检查(2.56%,2/78;30.77%,24/78;98.72%,77/78;87.18%,68/78)比较差异未见统计学意义(P>0.05);患侧腓总神经波幅、腓总神经F波出现率低于健侧(P<0.05)。结论EMG用于LDH定位诊断中具有较高准确率,且能弥补MRI检查的不足,判断神经传导异常情况,为诊断LDH和后续开展针对性治疗提供参考依据。ObjectiveTo study the significance of electroneuromyography(EMG)in the localization diagnosis of lumbar disc herniation(LDH).MethodsA retrospective study was conducted on 78 patients with LDH admitted to Shangqiu First People’s Hospital from January 2021 to December 2023.All patients underwent EMG and magnetic resonance imaging(MRI)examinations.The localization of the involved nerve segments in LDH diagnosed by EMG and MRI was statistically analyzed,and the nerve conduction indicators of EMG on the healthy and affected sides were compared.ResultsThe detection rates of EMG for the affected nerve segments of LDH at the 2nd to 3rd lumbar vertebrae(1.28%,1/78),at the 4th lumbar vertebrae(25.64%,20/78),at the 5th lumbar vertebrae(94.87%,74/78),and at the 1st sacral vertebrae(84.62%,66/78)had no significant difference with MRI(2.56%,2/78;30.77%,24/78;98.72%,77/78;87.18%,68/78),P>0.05.The amplitude of the common peroneal nerve and the occurrence rate of F-waves in the common peroneal nerve on the affected side were lower than those on the healthy side(P<0.05).ConclusionsEMG has a high accuracy in the localization diagnosis of LDH,and can compensate for the deficiencies of MRI examination.It can identify abnormalities in nerve conduction,and provide a reference basis for the diagnosis of LDH and subsequent targeted treatment.
