摘 要:目的探讨益生菌联合小麦纤维素颗粒治疗维持性血液透析(MHD)患者慢性便秘的临床效果。方法选取2022年3月至2024年5月连云港市立东方医院收治的合并慢性便秘的MHD患者108例作为研究对象,根据治疗方法不同分为观察组(55例)和对照组(53例)。对照组采用小麦纤维素颗粒治疗,观察组在对照组的基础上联合益生菌治疗。比较两组患者治疗前后的便秘严重程度和临床总有效率、焦虑和抑郁评分、炎症因子水平以及不良反应发生情况。结果治疗后观察组便秘患者症状自评量表(PAC-SYM)评分低于对照组,差异有统计学意义(P<0.001);观察组临床总有效率高于对照组,差异有统计学意义(P<0.05);治疗后观察组14项汉密尔顿焦虑量表(HAMA-14)、24项汉密尔顿抑郁量表(HAMD-24)评分低于对照组,差异有统计学意义(P<0.001);治疗后观察组白细胞介素-6(IL-6)、超敏C反应蛋白(hs-CRP)低于对照组,差异有统计学意义(P<0.05)。两组均未发生严重不良反应,两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论益生菌联合小麦纤维素颗粒治疗MHD患者的慢性便秘效果显著,在调节情绪的同时,还可以减少机体炎症反应,安全性高,方案简单易行。Objective To explore the therapeutic impact of a combination of probiotics and wheat cellulose particles in managing chronic constipation among patients undergoing maintenance hemodialysis(MHD).Methods A total of 108 MHD patients with chronic constipation admitted to Lianyungang Municipal Oriental Hospital from March 2022 to May 2024 were selected as the study objects,and they were divided into observation group(55 cases)and control group(53 cases)according to different treatment methods.The control group received treatment with wheat cellulose particles,while the observation group received probiotics in addition to wheat cellulose particles.The severity of constipation symptoms and the overall rate of clinical efficacy,anxiety and depression scores,levels of inflammatory factors,and occurrence of adverse reactions between the two groups before and after treatment were compared.Results After treatment,the patient assessment of constipation-symptoms(PAC-SYM)scores for constipation patients in the observation group were lower than those in the control group,with statistically significant differences(P<0.001).The overall effective rate of the observation group was higher than that of the control group,with statistically significant difference(P<0.05).Furthermore,the Hamilton anxiety scale-14(HAMA-14)and Hamilton depression scale-24(HAMD-24)in the observation group were lower than those in the control group,with statistically significant differences(P<0.001).The levels of interleukin-6(IL-6)and high-sensitivity creactive protein(hs-CRP)in the observation group were lower than those in the control group,with statistically significant differences(P<0.05).There was no serious adverse reaction between the two groups,and there was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Probiotics combined with wheat cellulose particles has significant therapeutic effect on chronic constipation among MHD patients by regulating emotions and reducing inflammation respon
