硝苯地平缓释片联合参麦注射液对妊娠高血压患者血压水平及胎盘血流指标的影响

摘　　要：目的探讨硝苯地平缓释片联合参麦注射液对妊娠高血压患者血压水平及胎盘血流指标的影响。方法选取2022年1月至2023年1月赣州市妇幼保健院收治的89例妊娠高血压患者作为研究对象,按照随机数字表法分为试验组(44例)、对照组(45例)。对照组予以硝苯地平缓释片治疗,试验组在对照组方法基础上联合参麦注射液治疗,比较两组的血压水平、胎盘血流指标、炎症因子水平、不良妊娠结局,以及不良反应发生情况。结果治疗后,试验组的血压[舒张压(DBP)、收缩压(SBP)]、胎盘血流指标[脐动脉搏动指数(PI)、收缩末期峰值/舒张末期峰值(S/D)、阻力指数(RI)]、炎症因子[γ干扰素(IFN-γ)、白细胞介素-17(IL-17)、肿瘤坏死因子-α(TNF-α)]水平均低于对照组,差异有统计学意义(P<0.05)。试验组的不良妊娠结局总发生率低于对照组,差异有统计学意义(P<0.05)。治疗期间,两组均未见明显不良反应。结论硝苯地平缓释片联合参麦注射液治疗妊娠高血压,能降低患者血压,改善胎盘血流灌注,减轻炎症反应,改善妊娠结局,且安全性良好。Objective To investigate the effect of Nifedipine Sustained-Release Tablets combined with Shenmai Injection on blood pressure level and placental blood flow index in patients with gestational hypertension.Methods A total of 89 patients with pregnancy-induced hypertension admitted to Ganzhou Women and Children's Health Care Hospital from January 2022 to January 2023 were selected as the research objects.According to the random number table method,they were divided into the experimental group(44 cases)and the control group(45 cases).The control group was treated with Nifedipine Sustained-Release Tablets,and the experimental group was treated with Shenmai Injection on the basis of the control group.The blood pressure level,placental blood flow index,inflammatory factor level,adverse pregnancy outcome,and adverse reactions were compared between the two groups.Results After treatment,the blood pressure(diastolic blood pressure[DBP],systolic blood pressure[SBP]),placental blood flow indexes(umbilical artery pulsatility index[PI],peak systolic flow velocity/end-diastolic flow velocity[S/D],resistance index[RI]),inflammatory factor levels(interferon-γ[IFN-γ],interleukin-17[IL-17],tumor necrosis factor-α[TNF-α])in the experimental group were lower than those in the control group,with statistically significant differences(P<0.05).The total incidence of adverse pregnancy outcomes in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).During the treatment,there were no obvious adverse reactions in the two groups.Conclusion Nifedipine Sustained-Release Tablets combined with Shenmai Injection in the treatment of gestational hypertension can reduce blood pressure,improve placental blood perfusion,reduce inflammatory response,and improve pregnancy outcomes,with good safety.
