中医五音疗法联合枸橼酸咖啡因对早产儿呼吸暂停的影响

摘　　要：目的分析中医五音疗法联合咖啡因对早产儿呼吸暂停的影响。方法选取2023年2月至2024年6月江西省妇幼保健院收治的216例早产儿作为研究对象,按照随机数字表法将其分为对照组、试验Ⅰ组与试验Ⅱ组,每组各72例,对照组采用低剂量枸橼酸咖啡因治疗,试验Ⅰ组采用低剂量枸橼酸咖啡因加中医五音疗法治疗,试验Ⅱ组采用高剂量枸橼酸咖啡因加中医五音疗法治疗。比较三组早产儿的生后7 d呼吸暂停指标、治疗有关指标,以及不良反应发生情况。结果三组患儿的生后7 d呼吸暂停发生率比较,差异有统计学差异(P<0.05)。试验Ⅱ组生后7 d每日呼吸暂停发生次数少于试验Ⅰ组,呼吸暂停消失、无创通气、氧疗、住院时间均短于试验Ⅰ组,差异有统计学意义(P<0.05)。试验Ⅰ组生后7 d每日呼吸暂停发生次数少于对照组,呼吸暂停消失、无创通气、氧疗、住院时间均短于对照组,差异有统计学意义(P<0.05)。三组的各不良反应发生率比较,差异无统计学意义(P>0.05)。结论中医五音疗法加高剂量枸橼酸咖啡因对早产儿呼吸暂停防治效果理想,安全性较高。Objective To analyze the effect of traditional Chinese medicine five-tone therapy combined with Caffeine Citrate on apnea in premature infants.Methods A total of 216 premature infants admitted to Jiangxi Maternal and Child Health Hospital from February 2023 to June 2024 were selected as the research objects.According to the random number table method,they were divided into control group,experimental groupⅠand experimental groupⅡ,with 72 cases in each group.The control group was treated with low-dose Caffeine Citrate,the experimental groupⅠwas treated with low-dose Caffeine Citrate combined with traditional Chinese medicine five-tone therapy,and the experimental groupⅡwas treated with high dose Caffeine Citrate and traditional Chinese medicine five-tone therapy.The apnea indicators 7 d after birth,treatment-related indicators,and adverse reactions were compared among the three groups.Results There was a statistically significant difference in the incidence of apnea on 7 d after birth among the three groups(P<0.05).The daily occurrence of apnea in experimental groupⅡwas less than that in experimental groupⅠat 7 d after birth,and the disappearance of apnea,non-invasive ventilation,oxygen therapy and hospital stay were shorter than those in experimental groupⅠ,with statistically significant differences(P<0.05).The daily incidence of apnea in the experimental groupⅠwas less than that in the control group during the first 7 d of life,and the disappearance of apnea,non-invasive ventilation,oxygen therapy,and hospitalization time were shorter than those in the control group,and the differences were statistically significant(P<0.05).There were no significant differences in the incidences of adverse reactions among the three groups(P>0.05).Conclusion Traditional Chinese medicine five-tone therapy combined with high dose of Caffeine Citrate is effective and safe in the prevention and treatment of apnea in preterm infants.
