摘 要:目的探讨鼻间歇正压通气联合枸橼酸咖啡因在呼吸暂停早产儿治疗中的效果。方法选取2022年6月至2024年3月莆田市第一医院收治的185例呼吸暂停早产儿作为研究对象,按照随机数字表法分为对照组(92例)与观察组(93例),对照组患儿采用持续正压通气联合枸橼酸咖啡因治疗,观察组患儿采用鼻间歇正压通气联合枸橼酸咖啡因治疗。比较两组患儿的监测指标水平、肺功能水平、呼吸机参数及早产儿视网膜病、支气管肺发育不良的发生率。结果观察组的每日呼吸暂停次数少于对照组,用氧时间、呼吸机使用时间、住院时间短于对照组,差异有统计学意义(P<0.05)。治疗后,观察组的各肺功能指标水平均高于对照组,差异有统计学意义(P<0.05)。治疗后,观察组的各呼吸机参数指标水平均低于对照组,差异有统计学意义(P<0.05)。观察组的早产儿视网膜病、支气管肺发育不良发生率均低于对照组,差异有统计学意义(P<0.05)。结论鼻间歇正压通气联合枸橼酸咖啡因治疗对改善呼吸暂停早产儿病情有着显著效果,可减少其呼吸暂停次数,优化呼吸机参数指标,同时有效降低早产儿视网膜病、支气管肺发育不良发生率,临床上可借鉴及推广。Objective To explore the effect of nasal intermittent positive pressure ventilation combined with Caffeine Citrate in the treatment of premature infants with apnea.Methods A total of 185 preterm infants with apnea admitted to the First Hospital of Putian City from June 2022 to March 2024 were selected as the research objects,and they were divided into control group(92 cases)and observation group(93 cases)according to the random number table method.The control group was treated with continuous positive pressure ventilation combined with Caffeine Citrate,while the observation group was treated with nasal intermittent positive pressure ventilation combined with Caffeine Citrate.The levels of monitoring indicators,lung function,ventilator parameters,and the incidence rates of retinopathy of prematurity and bronchopulmonary dysplasia were compared between the two groups.Results The number of daily apnea in the observation group was less than that in the control group,and the oxygen time,ventilator use time and hospitalization time were shorter than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the levels of pulmonary function indexes in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the levels of ventilator parameters in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The incidences of retinopathy of prematurity and bronchopulmonary dysplasia in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Nasal intermittent positive pressure ventilation combined with Caffeine Citrate treatment has a significant effect on the improvement of apnea in premature infants,reducing the number of apnea,optimizing the ventilator parameters,and effectively reduce the incidence of retinopathy of prematurity and bronchopulmonary dyspla
