摘 要:目的分析嗜酸性粒细胞(Eos)、白细胞计数(WBC)、C反应蛋白(CRP)联合检测对输血后发生不良反应的临床应用价值。方法选取2021年4月至2023年7月濮阳市安阳地区医院接受输血治疗的患者为研究对象(n=126),根据患者输血后不良反应发生情况分为发生组及未发生组。比较两组临床一般资料及输血前后Eos、WBC、CRP水平,并分析各指标联合检测对输血后不良反应的预测价值及发生输血不良反应的危险度。结果发生组输血次数、Eos、CRP、WBC水平高于未发生组(P<0.05);输血前后Eos、WBC、CRP水平联合预测患者发生不良反应的曲线下面积(AUC)为0.745、0.788,高于各指标单独预测;输血前血清Eos、WBC、CRP高水平患者发生输血不良的危险度是低水平的2.236、2.542、2.188倍,输血后血清Eos、WBC、CRP高水平患者发生输血不良的危险度是低水平的3.000、3.960、3.436倍(P<0.05)。结论Eos、WBC、CRP水平联合检测可对评估患者输血后不良反应进行有效预警,有利于疾病的早期诊治,值得临床推广应用。【Objective】To analyze the predictive value of combined detection of eosinophil(Eos),white blood cell count(WBC)and C-reactive protein(CRP)for adverse reactions after blood transfusion.【Methods】Patients who received blood transfusion in Anyang District Hospital from April 2021 to July 2023 were selected as the research objects(n=126).According to the incidence of adverse reactions after blood transfusion,the patients were divided into the occurrence group and the non-occurrence group.The clinical general data and the levels of Eos,WBC and CRP before and after blood transfusion were compared between the two groups,and the predictive value of the combined detection of each index on post-transfusion adverse reactions and the risk of occurrence of post-transfusion adverse reactions were analyzed.【Results】The times of blood transfusion and the levels of Eos,CRP and WBC in the occurrence group were significantly higher than those in the non-occurrence group(P<0.05).The area under the curve(AUC)of the combined prediction of adverse reactions in patients before and after blood transfusion using Eos,WBC,and CRP levels was 0.745 and 0.788,which was significantly higher than that of each indicator alone.The risk of adverse reactions in patients with high levels of serum Eos,WBC,and CRP before blood transfusion was 2.236,2.542,and 2.188 times higher than that of patients with low levels,respectively.The risk of adverse reactions in patients with high levels of serum Eos,WBC,and CRP after blood transfusion was 3.000,3.960,and 3.436 times higher than that of patients with low levels,respectively(P<0.05).【Conclusion】The combined detection of the levels of Eos,WBC and CRP can effectively predict the adverse reactions of patients after blood transfusion,which is conducive to the early diagnosis and treatment of the disease,and is worthy of clinical application.
