中医药有效成分经Th17/Treg细胞平衡改善炎症性肠病的研究进展

摘　　要：炎症性肠病(IBD)是一种慢性非特异性免疫疾病。目前IBD发病机制不明,治疗有局限性。临床表明,IBD发病受环境、遗传、免疫因素影响。研究发现,IBD患者肠道内具有明显的免疫功能紊乱,其中Th17细胞的异常增多和Treg细胞数量持续降低是其典型症状之一。免疫细胞的持续失衡将导致IBD的症状加重且不易治愈。在临床治疗IBD的过程中发现,调节患者肠道内Th17/Treg细胞的平衡可有效减轻IBD相关症状,并提高疾病的治愈率。因此,有学者认为调节Th17/Treg细胞平衡,恢复肠道免疫功能,可作为缓解IBD患者症状的有效治疗靶点。目前,通过调节免疫治疗IBD的西药存在疗效差、副作用较大且价格较为昂贵等缺点。而中药具有作用靶点广、价格较低廉、原料易获得等优势。研究表明,许多中药复方及中药天然产物内的效应成分具有显著的免疫调节作用,并可通过调节Th17细胞与Treg细胞的平衡来改善IBD的症状。本文就近年来中药复方以及中药天然产物中有效成分通过调节Th17、Treg细胞的平衡来治疗IBD的研究进展进行综述。Inflammatory bowel disease(IBD)is a chronic non-specific immune disorder.The pathogenesis of IBD is not clear at present,and the treatment is limited.Clinical studies have shown that IBD development is influenced by environmental,genetic,and immune factors.Research has found that IBD patients have obvious immune function disorders in their intestines,characterized by an abnormal increase in T helper cell 17(Th17)cells and a sustained decrease in regulatory T cell(Treg)cell numbers,which are typical symptoms.The persistent imbalance of immune cells will exacerbate the symptoms of IBD and make it difficult to cure.During clinical treatment of IBD,it was found that regulating the balance of Th17/Treg cells in the intestines of patients can effectively alleviate IBD-related symptoms and improve the cure rate of the disease.Therefore,some scholars believe that regulating the balance of Th17/Treg cells and restoring intestinal immune function can be an effective treatment target for alleviating the symptoms of IBD patients.Currently,Western medicines for regulating immunity to treat IBD have the disadvantages of poor efficacy,significant side effects,and relatively high prices.Traditional Chinese medicine has the advantages of wide-ranging action targets,lower prices,and easy access to raw materials.Studies have shown that many active ingredients in TCM possess significant immune-modulating effects and can improve IBD symptoms by regulating the balance between Th17 and Treg cells.This review summarizes the recent research progress on the effective components of herbal compounds and natural products in TCM that regulate Th17/Treg cell balance to treat IBD.
