摘　　要：目的分析血清金属肽酶含血小板反应蛋白原-1(ADAMTS-1)与成人病毒性心肌炎(VMC)心肌纤维化进程的关联。方法回顾性分析商丘市第一人民医院2021年10月至2024年1月收治的100例VMC成人患者的病例资料,分为轻型组(40例)、中型组(30例)、重型组(30例),比较3组ADAMTS-1、ADAMTS-1 mRNA,心肌纤维化指标,检验ADAMTS-1、ADAMTS-1 mRNA与心肌纤维化指标的相关性;验证ADAMTS-1、ADAMTS-1 mRNA对VMC患者心肌纤维化病变的预测价值。结果重型组的ADAMTS-1表达水平、ADAMTS-1 mRNA相对表达量均高于中型组、轻型组,差异有统计学意义(P<0.05);重型组的CTGF、LN、HA、ALD均高于中型组、轻型组(P<0.05);ADAMTS-1水平与CTGF、LN、HA、ALD均呈正相关(P<0.05);ADAMTS-1 mRNA相对表达量与CTGF、LN、HA、ALD均呈正相关(P<0.05);ADAMTS-1水平及ADAMTS-1 mRNA相对表达量联合检测对VMC患者心肌纤维化的预测灵敏度、特异度均高于ADAMTS-1、ADAMTS-1 mRNA单独检测。结论联合检测ADAMTS-1表达水平及ADAMTS-1 mRNA相对表达量能实现对患者心肌纤维化病变的早期预测。Objective To analyze the association between serum metallopeptidase containing platelet-derived reactive protein-1(ADAMTS-1)and the progression of myocardial fibrosis in adult viral myocarditis(VMC).Methods A retrospective analysis of case data for 100 adult patients with VMC admitted to the First People′s Hospital of Shangqiu City from October 2021 to January 2024 was conducted and divided them into the mild group(40 cases),the moderate group(30 cases),and the severe group(30 cases).ADAMTS-1,ADAMTS-1 mRNA,and myocardial fibrosis indicators were compared among the three groups,and the correlation between ADAMTS-1,ADAMTS-1 mRNA,and myocardial fibrosis indicators was examined.Verify the predictive value of ADAMTS-1 and ADAMTS-1 mRNA for myocardial fibrosis in VMC patients.Result The expression level and relative expression level of ADAMTS-1 mRNA in the severe group were higher than those in the moderate and mild groups,showing a statistically significant difference(P<0.05).The levels of CTGF,LN,HA,and ALD in the severe group were higher than those in the moderate and mild groups(P<0.05).ADAMTS-1 levels were positively correlated with CTGF,LN,HA,and ALD(P<0.05).The relative expression level of ADAMTS-1 mRNA was positively correlated with CTGF,LN,HA,and ALD(P<0.05).The combined detection of ADAMTS-1 levels and ADAMTS-1 mRNA relative expression levels has higher sensitivity and specificity in predicting myocardial fibrosis in VMC patients than the detection of ADAMTS-1 and ADAMTS-1 mRNA alone.Conclusion Joint detection of ADAMTS-1 expression levels and ADAMTS-1 mRNA relative expression levels can achieve early prediction of myocardial fibrosis lesions in patients.
