摘 要:目的分析微小RNA-23b-3p(miR-23b-3p)、微小RNA-223(miR-223)与重症肺炎病情进展及预后的关联。方法选取92例重症肺炎患者,将CURB-65得分≥3分的22例患者列为高危组,CURB-65得分为1~2分的30例患者列为中危组,CURB-65得分<1分的40例患者列为低危组,比较3组的血清miR-23b-3p、miR-223及肺功能;分析血清miR-23b-3p、miR-223与肺功能指标的相关性及对重症肺炎不良预后的预测效能。结果高危组的血清miR-23b-3p、FEV_(1)、FVC、PEF、MVV均低于中危组、低危组,miR-223高于中危组、低危组,差异有统计学意义(P<0.05);血清miR-23b-3p与FEV_(1)、FVC、PEF、MVV正相关(P<0.05),血清miR-223与FEV_(1)、FVC、PEF、MVV负相关(P<0.05);92例重症肺炎的不良预后发生率为21.74%(20/92);预后不良组的血清miR-23b-3p、FEV_(1)、FVC、PEF、MVV均低于预后良好组,miR-223高于预后良好组(P<0.05);血清miR-23b-3p对重症肺炎不良预后的预测灵敏度、特异度分别为83.11%、79.25%,血清miR-223对重症肺炎不良预后的预测灵敏度、特异度分别为81.22%、77.36%(AUC>0.85)。结论动态监测血清miR-23b-3p、miR-223水平变化能实现对患者不良预后的早期预测。Objective To analyze the association between microRNA-23b-3p(miR-23b-3p),microRNA-223(miR-223)and the progression and prognosis of severe pneumonia.Methods 92 patients with severe pneumonia were selected.Among them,22 patients with CURB-65 scores>3 were classified as the high-risk group,30 patients with CURB-65 scores 1 to 2 were classified as the medium-risk group,and 40 patients with CURB-65 scores<1 were classified as the low-risk group.The serum levels of miR-23b-3p,miR-223,and lung function indicators of the three groups were compared.The correlations between serum miR-23b-3p,miR-223,and lung function indicators,as well as their predictive efficacy for poor prognosis of severe pneumonia were analyzed,respectively.Results The serum levels of miR-23b-3p,FEV_(1),FVC,FEF,and MVC in the high-risk group were all lower than those in the medium-risk and low-risk groups.The miR-223 levels were higher than those in the medium-risk and low-risk groups.The differences were statistically significant(P<0.05).The serum miR-23b-3p was positively correlated with FEV_(1),FVC,PEF,and MVV(P<0.05).Serum miR-223 was negatively correlated with FEV_(1),FVC,PEF,and MVV(P<0.05).The incidence of adverse prognosis in 92 cases of severe pneumonia was 21.74%(20/92).The serum levels of miR-23b-3p,FEV_(1),FVC,PEF,and MVV in the poor prognosis group were all lower than those in the good prognosis group.The miR-223 levels were higher than those in the good prognosis group(P<0.05).The prediction sensitivity and specificity of serum miR-23b-3p for the poor prognosis of severe pneumonia were 83.11%and 79.25%,respectively.The prediction sensitivity and specificity of serum miR-223 for the poor prognosis of severe pneumonia were 81.22%and 77.36%,respectively(AUC>0.85).Conclusion Dynamic monitoring of changes in serum miR-23b-3p and miR-223 levels can achieve an early prediction of poor prognosis in patients with severe pneumonia.
