摘 要:目的分析血清微小RNA-181a(miR-181a)、1-磷酸鞘氨醇受体1(S1PR1)与支气管哮喘(BA)病情严重程度及肺损伤程度的关联。方法回顾性分析我院收治的108例BA患者的病例资料,依据病情严重程度不同分为重度组(28例)、中度组(40例)、轻度组(40例),检验血清miR-181a、S1PR1与炎症指标、肺功能指标以及评分的相关性及对BA患者病情进展情况的预测效能。结果重度组的miR-181a低于中度组、轻度组,SIPR1高于中度组、轻度组,差异有统计学意义(P<0.05)。重度组的Neu、EOS、FeNO均高于中度组、轻度组(P<0.05)。重度组的FEV1、FVC、MVV、ACT评分均低于中度组、轻度组(P<0.05)。血清miR-181a与Neu、EOS、FeNO负相关,与FEV1、FVC、MVV、ACT评分正相关(P<0.05);血清SIRP1与Neu、EOS、FeNO正相关,与FEV1、FVC、MVV、ACT评分负相关(P<0.05)。miR-181a、S1RP1联合检测对BA病情进展的预测灵敏度、特异度均高于miR-181a单独检测、S1RP1单独检测。结论联合检测miR-181a、S1RP1能实现对患者病情严重程度及肺功能损伤程度的早期预测。Objective To analyze the correlation between micro RNA-181a(miR-181a),1-phosphate sphingosine receptor 1(S1PR1)and the severity of illness and degree of lung injury in the patients with the bronchial asthma(BA).Methods A retrospective analysis was conducted on the case data of 108 BA patients at Puyang Huimin Hospital.The patients were divided into the severe group(28 cases),the moderate group(40 cases),and the mild group(40 cases)based on the severity of their condition.The correlation between serum miR-181a,S1PR1 and the inflammation indicators,lung function indicators,and scores,as well as their predictive efficiency for the progression of BA patients,were examined.Results The miR-181a levels in the severe group were lower than those in the moderate and mild groups,while the S1PR1 level was higher than that in the moderate and mild groups,showing a statistically significant difference(P<0.05).The Neu,EOS,and FeNO levels in the severe group were higher than those in the moderate and mild groups(P<0.05).The FEV1,FVC,MVV,and ACT scores in the severe group was lower than that in the moderate and mild groups(P<0.05).Serum miR-181a was negatively correlated with the levels of Neu,EOS,and FeNO,and was positively correlated with the FEV1,FVC,MVV,and ACT scores(P<0.05).Serum SIRP1 was positively correlated with the levels of Neu,EOS,and FeNO,and was negatively correlated with the FEV1,FVC,MVV,and ACT scores(P<0.05).The combined detection of miR-181a and S1RP1 had higher sensitivity and specificity in predicting the progression of BA patients than detecting miR-181a alone or S1RP1 alone.Conclusion Combined detection of miR-181a and S1RP1 achieves early prediction of the severity of illness and the degree of lung function damage in the patients with BA.
