摘　　要：目的分析微小RNA-210(miR-210)、微小RNA-181b(miR-181b)与妊娠期高血压(HDP)病情进展及预后情况的关联。方法选取125例HDP患者,分为轻度组(45例)、中度组(40例)、重度组(40例),分析其血清miR-210、miR-181b与HDP血压水平及炎症指标间的相关性及对HDP不良预后的预测效能。结果重度组的miR-210、miR-181b、收缩压(SBP)、舒张压(DBP)、平均动脉压(MAP)均高于中度组,轻度组,差异具有统计学意义(P<0.05)。重度组的白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、高迁移率族蛋白B1(HMGB1)均高于中度组,轻度组(P<0.05)。miR-210与SBP、DBP、MAP、IL-6、IL-8、HMGB1正相关(P<0.05);miR-181b与SBP、DBP、MAP、IL-6、IL-8、HMGB1正相关(P<0.05)。预后不良组的miR-210、miR-181b、IL-6、IL-8、HMGB1均高于预后良好组(P<0.05)。miR-210、miR-181b联合检测对HDP不良妊娠结局的预测灵敏度、特异度均高于miR-210单独检测、miR-181b单独检测。结论联合检测miR-210、miR-181b能实现对患者不良妊娠结局的早期预测。Objective To analyze the association between miR-210,miR-181b and the progression and prognosis of the hypertensive disorders in pregnancy(HDP).Methods 125 patients with HDP were divided into the mild group(45 cases),the moderate group(40 cases),and the severe group(40 cases).The correlation between serum miR-210,miR-181b and the blood pressure levels and the inflammatory indicators,as well as their predictive efficiency for poor prognosis of HDP,were analyzed.Results The miR-210,miR-181b,systolic blood pressure(SBP),diastolic blood pressure(DBP),and mean arterial pressure(MAP)in the severe group were all higher than those in the moderate group and the mild group(P<0.05).The levels of interleukin-6(IL-6),interleukin-8(IL-8),and high mobility group protein B1(HMGB1)in the severe group were higher than those in the moderate group and the mild group(P<0.05).The levels of miR-210 was positively correlated with the SBP,DBP,MAP,IL-6,IL-8,and HMGB1(P<0.05).The levels of miR-181b was positively correlated with the SBP,DBP,MAP,IL-6,IL-8,and HMGB1(P<0.05).The miR-210,miR-181b,IL-6,IL-8,and HMGB1 levels in the poor prognosis group were higher than those in the good prognosis group(P<0.05).The combined detection of miR-210 and miR-181b had higher sensitivity and specificity in predicting adverse pregnancy outcomes in HDP compared to the detection of miR-210 and miR-181b alone.Conclusion The combined detection of miR-210 and miR-181b achieves early prediction of adverse pregnancy outcomes in THE patients with HDP.
