摘　　要：产后缺乳属中医“乳汁不下”“乳汁不行”等范畴,指产妇在产褥期、哺乳期内乳汁正行之际,乳汁分泌不足以喂养婴儿。现代医学认为,产后缺乳与乳房的发育及催乳素的调控机制密切相关。中医学认为气血亏虚、冲任失调、肾精不足造成的乳汁化生不足和气滞、痰湿、血瘀导致的乳汁运行不畅是产后缺乳的主要病因病机。楼丽华教授系浙江省国医名师,临证数十载,创立了自己独特的产后缺乳诊治思路。楼教授认为产后缺乳以虚证为主,其本质为虚和寒,妇人生产时耗气伤血,阳气耗散,血虚则乳汁生化乏源,气虚、阳虚则推动无力,寒凝乳络,故乳汁难排。楼教授通过产前对产妇的情绪、基础疾病的调控预防该病的发生,对产后缺乳患者的临床治疗以补益气血、温阳通乳为法,以八珍汤合阳和汤加减为方,同时辅以哺乳方式及饮食生活指导,疗效良好。本文主要介绍楼丽华教授治疗产后缺乳的临床经验。Postpartum hypogalactia falls under the traditional Chinese medicine categories of"breast milk stoppage"and"galactostasis",which refers to the milk is not enough to feed the baby during the puerperal and lactation periods.Modern medicine believes that postpartum hypogalactia is closely related to breast development and the regulation mechanism of prolactin.In traditional Chinese medicine,the primary causes of postpartum hypogalactia are insufficient milk metaplasia caused by qi and blood deficiency,disharmony of Chong and Ren channels,insufficiency of kidney essence,and obstructed milk discharge due to qi stagnation,phlegm dampness,and blood stasis.Professor LOU Lihua,a renowned traditional Chinese medicine practitioner in Zhejiang Province with decades of clinical experience,has developed unique diagnostic and therapeutic approaches for postpartum hypogalactia.Professor LOU posits that postpartum hypogalactia primarily manifests as a deficiency syndrome,characterized by deficiency and coldness.During childbirth,women often suffer from qi and blood depletion,yang dissipation.Blood deficiency leads to insufficient sources for milk production.Qi and yang deficiencies weaken the body's ability to promote,while cold congealed in the breast veins hinders milk flow.To prevent this condition,Professor Lou emphasizes the regulation of maternal emotions and management of pre-existing conditions before childbirth.In clinical practice,she employs methods of tonifying qi and blood,warming yang,and promoting milk secretion using modified Bazhen Decoction combined with Yanghe Decoction.Additionally,she provides feeding method guidance and dietary recommendations,achieving satisfactory results.This article mainly introduces Professor LOU Lihua's clinical experience in treating postpartum hypogalactia.
