摘　　要：目的探讨自制龙中冰茶外敷对糖尿病肾病透析患者自体动静脉内瘘的影响。方法选取2022年1月至2023年6月厦门大学附属龙岩中医院收治的68例糖尿病肾病透析患者作为研究对象,采用随机抛币法分为对照组和观察组,各34例。对照组采用常规护理方案进行干预,观察组在对照组的基础上联合自制龙中冰茶外敷。观察两组患者干预前后动静脉内瘘血液流速、血管壁厚度、静脉血管内径、瘘口面积、生活质量评分及并发症发生率。结果干预后,两组患者动静脉内瘘血液流速、血管壁厚度、静脉血管内径、瘘口面积高于本组干预前,且观察组指标高于对照组,差异有统计学意义(P<0.05)。干预后,两组患者总体健康、生活职能、心理健康、社会功能、躯体疼痛、生理功能、活力、情感职能评分高于本组干预前,且观察组评分高于对照组,差异有统计学意义(P<0.05)。观察组并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论自制龙中冰茶外敷可有效维持糖尿病肾病透析患者动静脉内瘘血液流速,促进动静脉内瘘的通畅,降低动静脉内瘘的并发症发生率,提高患者的生活质量,值得临床借鉴。Objective To investigate the effect of external application of Self-made Longzhong Ice Tea on autologous arteriovenous fistula in patients with diabetic nephropathy on dialysis.Methods A total of 68 patients on dialysis with diabetic nephropathy admitted to the Longyan Hospital of TCM Affiliated to Xiamen University from January 2022 to June 2023 were selected as the study objects.They were divided into control group and observation group by random coin tossing method,with 34 patients in each group.The control group was treated with routine nursing regimen,and the observation group was treated with external application of Self-made Longzhong Ice Tea on the basis of the control group.Blood flow rate,wall thickness,inner diameter,orifice area,quality of life score and complication rate of arteriovenous fistula in two groups were observed before and after intervention.Results After intervention,the blood flow rate,vessel wall thickness,venous diameter and fistula opening area of the two groups were higher than those before intervention,and the indexes of observation group were higher than those of the control group,the differences were statistically significant(P<0.05).After intervention,the scores of overall health,life function,mental health,social function,physical pain,physiological function,vitality and emotional function in two groups were higher than those before intervention,and the scores of observation group were higher than those of the control group,the differences were statistically significant(P<0.05).The total incidence of complications in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion External application of Self-made Longzhong Ice Tea can effectively maintain the blood flow rate of arteriovenous fistula in dialysis patients with diabetic nephropathy,promote the patency of arteriovenous fistula,reduce the incidence of complications of arteriovenous fistula,and improve the quality of life of patients,which is worth clinica
