微通道经皮肾镜取石术联合负压吸引清石鞘在复杂性肾结石治疗中的临床效果研究

摘　　要：目的探究在复杂性肾结石治疗中实施微通道经皮肾镜取石术(mPCNL)+负压吸引清石鞘治疗的效果。方法80例复杂性肾结石患者作为受试者,以数表法随机分为对照组和观察组,每组40例。对照组行mPCNL治疗,观察组行mPCNL、负压吸引清石鞘联合治疗。比较两组患者手术相关指标,肾血流、肾功能指标,并发症发生情况,结石一期清除成功率。结果观察组患者手术时间(112.56±10.70)min、术中出血量(59.02±8.31)ml、胃肠道功能恢复时间(0.71±0.23)d、住院时间(11.50±3.24)d比对照组的(143.29±15.16)min、(77.83±6.15)ml、(0.95±0.29)d、(16.84±4.05)d更优(P<0.05)。术后,观察组患者阻力指数(RI)(0.68±0.10)、最大血流速度(Vmax)(74.78±7.22)cm/s、收缩期峰值流速与舒张末期流速的比值(S/D)(3.35±0.63)比对照组的(0.80±0.13)、(67.03±7.32)cm/s、(2.84±0.51)更优(P<0.05)。观察组并发症发生率10.00%比对照组的32.50%更低(P<0.05)。观察组结石一期清除成功率97.50%比对照组的80.00%更高(P<0.05)。结论将mPCNL、负压吸引清石鞘联合用于复杂性肾结石治疗中疗效理想,可缓解肾脏受损程度,促进肾血流恢复,提升结石一期清除成功率,减少术后并发症发生,促进患者术后恢复,值得推广。Objective To explore the effect of microchannel percutaneous nephrolithotomy(mPCNL)combined with ClearPetra access sheath with vacuum suction in the treatment of complex kidney stones.Methods 80 patients with complex kidney stones as subjects were divided into control group and observation group by random numerical table,with 40 cases in each group.The control group was treated with mPCNL,and the observation group was treated with mPCNL combined with ClearPetra access sheath with vacuum suction.The surgery related indexes,renal blood flow,renal function indexes,occurrence of complications,and success rate of one-stage stone removal were compared between the two groups.Results In the observation group,the operation time was(112.56±10.70)min,the amount of intraoperative blood loss was(59.02±8.31)ml,the recovery time of gastrointestinal function was(0.71±0.23)d,and the length of hospital stay was(11.50±3.24)d,which were better than(143.29±15.16)min,(77.83±6.15)ml,(0.95±0.29)d,and(16.84±4.05)d in the control group(P<0.05).After surgery,the observation group had resistance index(RI)of(0.68±0.10),maximum blood flow velocity(Vmax)of(74.78±7.22)cm/s and ratio of peak systolic velocity to end diastolic velocity(S/D)of(3.35±0.63),which were better than(0.80±0.13),(67.03±7.32)cm/s and(2.84±0.51)in the control group(P<0.05).The complication rate of 10.00%in the observation group was lower than 32.50%in the control group(P<0.05).The success rate of one-stage stone removal in the observation group was 97.50%,which was higher than 80.00%in the control group(P<0.05).Conclusion The combination of mPCNL and ClearPetra access sheath with vacuum suction in the treatment of complex kidney stones has ideal efficacy,which can alleviate the degree of kidney damage,promote the recovery of kidney blood flow,improve the success rate of one-stage stone removal,reduce the occurrence of postoperative complications,and promote the postoperative recovery of patients,which is worthy of promotion.
