阶段性康复结合基于改良Harris-Benedict系数法的营养策略在重症颅脑损伤患者中的应用

摘　　要：目的分析阶段性康复结合基于改良Harris-Benedict(HB)系数法的营养策略在重症颅脑损伤患者中的应用效果。方法选取2023年1~11月滨州市中心医院收治的100例重症颅脑损伤患者,使用简单随机法分为观察组(50例)和对照组(50例),2组均接受综合护理和阶段性康复干预,观察组加用基于改良HB系数法的营养支持。对比2组的恢复指标和并发症发生率等。结果观察组的机械通气时间、肛门排气时间及住院时间均早于或短于对照组(P均<0.05)。干预1个月后,2组的白蛋白、总蛋白、血红蛋白均较干预前升高,C反应蛋白、降钙素原、白介素-6水平,以及美国国立卫生院神经功能缺损评分、日常生活能力量表评分均较干预前下降(P均<0.05);观察组在干预1个月后的上述指标水平和评分均较对照组更优(P均<0.05)。并发症总发生率的比较结果为,观察组更低(P<0.05)。结论在阶段性康复的同时加用基于改良HB系数法开展营养支持,能够促进重症颅脑损伤患者的早期康复,减少并发症的发生,且能够帮助患者改善营养指标、炎症反应、神经功能和日常生活能力。Objective To analyze the application effect of staged rehabilitation combined with nutrition strategy based on the modified Harris-Benedict(HB)equation in patients with severe craniocerebral injury.Methods One hundred patients with severe craniocerebral injury were enrolled from Binzhou Central Hospital between January 2023 and November 2023.They were divided into an observation group(50 cases)and a control group(50 cases)via simple randomization.Both groups received comprehensive nursing and staged rehabilitation intervention,while the observation group additionally received nutritional support based on the modified HB equation.The recovery indicators and the incidence of complications were compared between the two groups.Results The duration of mechanical ventilation,time to anal exhaust,and length of hospital stay of the observation group were shorter than those of the control group(P<0.05).After 1 month of intervention,the levels of albumin,total protein,and hemoglobin in both groups increased compared with those before intervention,and the levels of C-reactive protein,procalcitonin,and interleukin-6,as well as the scores of the National Institutes of Health Stroke Scale and the Activities of Daily Living Scale decreased compared with those before intervention(P<0.05).The observation group showed better levels and scores of the above indicators than the control group after 1 month of intervention(P<0.05).The comparison of the total incidence of complications showed that the observation group had a lower incidence(P<0.05).Conclusion The additional nutritional support based on the modified HB equation during stage rehabilitation can promote the early recovery of patients with severe craniocerebral injury,reduce the incidence of complications,and help improve the nutritional indicators,inflammatory reaction,neurological function,and activities of daily living.
