摘  要:目的探讨集束化管理小组在行体外膜肺氧合(ECMO)重症患者中的应用价值。方法纳入苏州市立医院于2020年1月至2023年12月收治的86例接受ECMO生命支持的重症患者,按简单随机抽样法分为观察组(43例)和对照组(43例),2组均接受常规护理,观察组组建集束化管理小组并实施集束化护理。对比2组的康复情况。结果观察组的ECMO上机决策沟通、用物准备、机器预冲和置管操作时间,以及ECMO辅助时间、ICU住院时间、总住院时间均较对照组更短,撤机成功率更高(P均<0.05)。与管理前比较,2组在管理7 d后的动脉血氧分压、氧合指数均上升,动脉血二氧化碳分压均下降(P均<0.05);与对照组比较,观察组在管理7 d后均更优(P均<0.05)。结论集束化管理小组能够显著缩短ECMO上机各环节耗时,降低ECMO相关并发症发生率,且对于加快康复进程和改善血气分析指标均有积极意义。Objective To explore the application value of cluster management team in critically ill patients receiving extracorporeal membrane oxygenation(ECMO).Methods A total of 86 critically ill patients who received ECMO life support at Suzhou Municipal Hospital from January 2020 to December 2023 were recruited and divided into an observation group(43 cases)and a control group(43 cases)using the simple random sampling.Both groups received routine nursing,and the observation group formed a cluster management team and implemented cluster nursing.The recovery status of the two groups were compared.Results The time required for decision-making communication of ECMO,material preparation,machine pre-flushing and cannulation,as well as the ECMO support time,ICU hospitalization time,and total hospitalization time in the observation group were shorter than those in the control group,and the success rate of ECMO weaning in the observation group was higher(P<0.05).Compared with the pre-management values,both groups showed an increase in arterial partial pressure of oxygen and oxygenation index and a decrease in arterial partial pressure of carbon dioxide after 7 days of management(P<0.05).Compared with the control group,the observation group showed better results after 7 days of management(P<0.05).Conclusion The cluster management team can significantly shorten the time required for various ECMO initiation procedures,reduce the incidence of ECMO-related complications,which has a positive impact on accelerating the recovery process and improving the blood gas analysis parameters.
