摘  要:目的构建食管癌围手术期内营养不良风险干预策略并考察应用效果。方法选取2021年4月至2024年4月在甘肃省武威肿瘤医院经手术治疗的130例食管癌患者,采用随机数字表法分为2组,每组65例,对照组采用常规护理方法,观察组构建围手术期内营养不良风险干预策略并实施应用。比较2组的术后康复时间、营养状况[患者主观整体评估量表(PG-SGA)、血红蛋白(Hb)、白蛋白(ALB)]、胃肠道激素水平[血清促胃液素(GAS)和胃动素(MTL)水平]、术后感染发生率、护理满意度及生活质量。结果观察组的住院、胃管留置、首次排气、肠鸣音恢复及首次进食用时均短于对照组(P均<0.05)。术后7 d和14 d,观察组的PG-SGA评分均低于对照组,Hb、ALB、GAS和MTL水平及生活质量评分均高于对照组(P均<0.05)。观察组的术后感染发生率低于对照组,护理满意度评分高于对照组(P均<0.05)。结论构建及应用食管癌患者围手术期内营养不良风险干预策略,有助于改善患者的术后营养状况和生活质量,加速康复,减少感染,促进胃肠道功能恢复,且患者的护理满意度较高。Objective To investigate the construction and application effect of malnutrition risk intervention strategy in the perioperative period of esophageal cancer.Methods A total of 130 esophageal cancer patients who underwent surgical treatment at Gansu Wuwei Tumour Hospital from April 2021 to April 2024 were selected and divided into two groups by using a random number table,with 65 patients in each group.The control group received conventional nursing method,while the observation group developed and adopted a perioperative malnutrition risk intervention strategy.The postoperative recovery time,nutritional status(Patient-Generated Subjective Global Assessment[PGSGA],hemoglobin[Hb],and albumin[ALB]),gastrointestinal hormone(gastrin[GAS],motilin[MTL])levels,incidence of postoperative infections,nursing satisfaction,and quality of life were compared between the two groups.Results The time of hospitalization,gastric tube indwelling,first exhaust,recovery of bowel sounds,and first food intake of the observation group were all shorter than those of the control group(P<0.05).At 7 days and 14 days after surgery,the PG-SGA score of the observation group was both lower than that of the control group,and the Hb,ALB,GAS,MTL levels and quality of life score were all higher than those of the control group(P<0.05).The incidence of postoperative infections in the observation group was lower than that in the control group,and the nursing satisfaction score was higher than that in the control group(P<0.05).Conclusion The construction and application of perioperative malnutrition risk intervention strategy for esophageal cancer patients can help improve the postoperative nutritional status and quality of life,accelerate the recovery,reduce the infections,promote the gastrointestinal function recovery,and increase the patient satisfaction with nursing.
