情志护理结合耳穴揿针在湿热下注型混合痔患者术后康复中的应用

摘　　要：目的分析情志护理结合耳穴揿针在湿热下注型混合痔患者术后康复进程中的应用价值。方法2022年6月至2023年6月,选取112例混合痔患者开展前瞻性对照研究,均于苏州市中医医院接受治疗,使用简单随机抽样法将患者分为观察组和对照组,各56例。2组均接受常规护理联合情志护理,观察组加用耳穴揿针干预。对比2组的疼痛视觉模拟评分(VAS)、创面水肿评分、广泛性焦虑障碍量表(GAD-7)评分、患者健康问卷(PHQ-9)评分、创面愈合时间和创面愈合等级。结果观察组在术后12 h、术后1 d、术后3 d、术后5 d和术后7 d的VAS评分和创面水肿评分均较同期对照组更低(P均<0.05)。观察组在术后7 d时的GAD-7和PHQ-9评分均较对照组更低(P均<0.05)。观察组的创面较对照组愈合更早,创面愈合等级较对照组更优(P均<0.05)。结论在情志护理的基础上联合耳穴揿针,能够帮助湿热下注型混合痔患者缓解术后疼痛和创面水肿,改善其心理状态,缩短创面愈合时间,提高愈合质量。Objective To analyze the application value of emotional nursing combined with auricular acupoint pressing needle in the postoperative rehabilitation process of patients with mixed hemorrhoids of dampness-heat diffusing downward.Methods From June 2022 to June 2023,a prospective controlled study was conducted on 112 patients with mixed hemorrhoids,all of whom received treatment at Suzhou Hospital of Traditional Chinese Medicine.The patients were allocated into an observation group and a control group using simple random sampling,each with 56 patients.Both groups received routine nursing combined with emotional nursing,while the observation group received additional intervention of auricular acupoint pressing needle.The Visual Analogue Scale(VAS)score,wound edema score,7-item Generalized Anxiety Disorder Scale(GAD-7)score,Patient Health Questionnaire-9(PHQ-9)score,wound healing time,and wound healing grade were compared between the two groups.Results The VAS and wound edema scores of the observation group were lower than those of the control group at 12 hours,1 day,3 days,5 days,and 7 days after surgery(P<0.05).The GAD-7 and PHQ-9 scores of the observation group were lower than those of the control group at 7 days after surgery(P<0.05).The wounds in the observation group exhibited accelerated healing,with a superior wound healing grade when compared with the control group(P<0.05).Conclusion On the basis of emotional nursing,auricular acupoint pressing needle can help relieve the postoperative pain and wound edema of patients with mixed hemorrhoids of dampness-heat diffusing downward,improve the psychological state and healing quality,and shorten the wound healing time.
