中医五行音乐疗法结合耳穴贴压及中药熏洗坐浴在混合痔患者术后的应用

摘　　要：目的考察中医五行音乐疗法结合耳穴贴压及中药熏洗坐浴在混合痔患者术后的应用效果。方法选取在徐州医科大学附属医院行手术治疗的混合痔患者作为研究对象,共计100例,入组时间为2021年10月至2023年10月,通过随机数字表法将其分为对照组和观察组,每组50例。对照组给予常规护理方案,观察组给予中医五行音乐疗法结合耳穴贴压及中药熏洗坐浴方案。比较2组的术后肛缘水肿程度、疼痛程度、康复时间、睡眠质量、焦虑感及并发症情况。结果术后3 d和7 d,2组的肛缘水肿评分、视觉模拟评分法评分、匹兹堡睡眠质量指数评分和焦虑自评量表评分均低于术后1 d(P均<0.05),且观察组的上述评分均低于同一时间对照组(P均<0.05);2组在术后7 d的上述评分均比术后3 d更低(P均<0.05)。观察组的住院、创面愈合及水肿消失时间均较对照组更短或更早,并发症(如尿潴留、肛门出血等)总发生率更低(P均<0.05)。结论中医五行音乐疗法结合耳穴贴压及中医熏洗坐浴可有效缓解混合痔患者术后的水肿、疼痛感及焦虑感,缩短康复时间,降低并发症发生率,改善睡眠质量。Objective To investigate the application effect of traditional Chinese medicine fivephase music therapy combined with auricular plaster and traditional Chinese medicine fumigation and sitz bath in patients with mixed hemorrhoids after surgery.Methods One hundred patients with mixed hemorrhoids who underwent surgical treatment at the Affiliated Hospital of Xuzhou Medical University were recruited.Recruitment was done from October 2021 to October 2023 through random number table method.The patients were randomized into two groups:50 patients in the control group received routine nursing and 50 patients in the observation group received traditional Chinese medicine fivephase music therapy combined with auricular plaster and traditional Chinese medicine fumigation and sitz bath.The degree of postoperative anal margin edema,pain,recovery time,sleep quality,anxiety,and complications were compared between the two groups.Results On the 3rd and 7th postoperative days,the scores of anal margin edema,Visual Analogue Scale,Pittsburgh Sleep Quality Index,and Self-Rating Anxiety Scale in both groups were lower than those on the 1st postoperative day(P<0.05),and the observation group achieved lower scores than the control group in the corresponding time(P<0.05).Both groups achieved lower scores on the 7th postoperative day compared with those on the 3rd postoperative day(P<0.05).The time of hospitalization,wound healing and edema disappearance in the observation group was shorter than that in the control group,and the total incidence of complications(urinary retention,anal anal bleeding,etc.)was lower(P<0.05).Conclusion Traditional Chinese medicine five-phase music therapy combined with auricular plaster and traditional Chinese medicine fumigation and sitz bath can effectively alleviate the postoperative edema, pain, and anxiety in patients after surgery for mixed hemorrhoids, shorten the recovery time, reduce the incidence of complications, and improve the sleep quality.
