摘 要:目的建立老年人工髋关节置换术后基于时机理论和格林健康教育的高质量护理策略,并评价其应用价值。方法选取中山市黄圃人民医院2021年1月至2024年1月收治的90例拟行人工髋关节置换术治疗的老年患者,使用简单随机抽样法将其分别纳入观察组(45例)和对照组(45例),2组均接受常规护理,观察组建立基于时机理论和格林健康教育的高质量术后护理策略并应用。对比2组的视觉模拟评分法(VAS)评分、Harris髋关节功能评分、Barthel指数和自我护理能力量表(ESCA)评分差异。结果观察组在术后12 h、24 h、48 h和72 h时的VAS评分均较对照组更低(P均<0.05),2组均较术前降低(P均<0.05)。观察组在术后1周、1个月和3个月时的Harris髋关节功能评分、Barthel指数及ESCA评分均较同期对照组更高(P均<0.05),2组均较术前更高(P均<0.05)。结论建立基于时机理论和格林健康教育的高质量护理策略并应用于老年人工髋关节置换术后护理,能够帮助患者减轻术后疼痛,且有助于促进其髋关节功能改善,以及自理和自护能力的提高。Objective To establish a high-quality nursing strategy based on timing theory and PRECEDE-PROCEED health education model after the artificial hip replacement in the elderly,and to evaluate its application value.Methods Ninety elderly patients who underwent artificial hip replacement at Huangpu People's Hospital of Zhongshan from January 2021 to January 2024 were selected and randomly included into an observation group(45 cases)and a control group(45 cases)using a simple random sampling method.Both groups received routine nursing care,and a high-quality postoperative nursing strategy based on timing theory and PRECEDE-PROCEED health education model was established and applied in the observation group.The differences in Visual Analogue Scale(VAS)scores,Harris Hip Scores,Barthel Index,and Exercise of Self-Care Agency(ESCA)scores were compared between the two groups.Results The VAS scores of the observation group were lower than those of the control group at 12,24,48,and 72 h after surgery(P<0.05),and both groups showed a decrease compared with preoperative levels(P<0.05).The Harris Hip Scores,Barthel Index,and ESCA scores of the observation group were higher than those of the control group at 1 week,1 month,and 3 months after surgery(P<0.05),and both groups showed an increase compared with preoperative levels(P<0.05).Conclusion The establishment of high-quality nursing strategy based on timing theory and PRECEDE-PROCEED health education model and its application in nursing care of elderly patients after artificial hip replacement can help patients to relieve postoperative pain and contribute to the improvement of hip function and self-care ability.
