激素替代治疗结合阿仑膦酸钠对围绝经期OP患者E2、FSH及骨代谢水平的影响

摘　　要：目的分析激素替代治疗(hormone replacement therapy,HRT)结合阿仑膦酸钠对围绝经期骨质疏松症患者的影响。方法以本院收治的82例围绝经期骨质疏松症患者为研究对象,病例收集时间为2022年1月至2023年12月,分组方法为随机投掷法,分为观察组(HRT+阿仑膦酸钠治疗)和对照组(阿仑膦酸钠治疗),每组各41例。比较两组血清激素水平[雌二醇(E2)、促卵泡刺激素(FSH)]、骨代谢水平[骨特异性碱性磷酸酶(BAP)、抗酒石酸酸性磷酸酶-5b(TRAP-5b)、β-胶原特殊序列(β-CTX)、骨钙素(BGP)]、骨密度指标(腰椎L2-L4、两侧股骨颈)、治疗安全性。结果治疗6个疗程后,较之于对照组,观察组FSH水平及BAP、TRAP-5b、β-CTX、BGP水平均更低,E2水平及腰椎L2-L4、左侧股骨颈、右侧股骨颈的骨密度值均更高(P<0.05);两组药物不良反应发生率对比无统计学意义(7.32%vs 12.20%,χ^(2)=0.138,P=0.709>0.05)。结论对于围绝经期骨质疏松症患者来说,采取HRT结合阿仑膦酸钠进行治疗,不仅能有效调节E2、FSH水平,还能积极改善骨代谢水平,也能在一定程度上增加骨密度,且未见严重不良反应。Objective To analyze the effects of hormone replacement therapy(HRT)combined with alendronate sodium on patients with perimenopausal osteoporosis.Methods The study subjects were 82 cases of perimenopausal osteoporosis patients admitted to our hospital,the year of case collection was January 2022-December 2023,the grouping method was randomized throw method,and the grouping groups were the observation group(HRT+alendronate treatment)and the control group(alendronate treatment),with 41 cases in each group.Serum hormone levels[estradiol(E2),follicle-stimulating hormone(FSH)],bone metabolism levels[bone-specific alkaline phosphatase(BAP),anti-tartaric acid phosphatase-5b(TRAP-5b),β-collagen specific sequence(β-CTX),osteocalcin(BGP)],bone mineral density indexes(lumbar vertebrae L2-L4,femoral necks on both sides),and safety of treatment were compared between the two groups.Translated with DeepL.com(free version).Results After 6 courses of treatment,compared with the control group,the FSH level and the levels of BAP,TRAP-5b,β-CTX,and BGP were lower in the observation group,and the levels of E2 and the BMD values of lumbar vertebrae L2-L4,the left femoral neck,and the right femoral neck were higher(P<0.05);and the incidence rates of side effects of the medications were compared between the 2 groups(7.32%vs.12.20%,χ^(2)=0.138,P=0.709>0.05).Conclusion For patients with perimenopausal osteoporosis,HRT combined with alendronate sodium can not only effectively regulate the levels of E2 and FSH,but also actively improve the level of bone metabolism and increase bone mineral density to a certain extent,without serious adverse reactions.
