基于数据挖掘探讨近10年针灸治疗炎症性肠病的选穴规律

摘　　要：目的基于数据挖掘的方法,分析近10年针灸治疗炎症性肠病(IBD)的选穴规律。方法在PubMed、Embase、Cochrane Library、Web of Science、中国生物医学文献数据库和中国知网数据库中检索针灸治疗IBD相关临床研究文献,从中提取所采用的腧穴组成处方,并建立腧穴数据库,运用SPSS 26.0软件对腧穴使用频次、所属经脉分布以及特定腧穴运用情况进行统计分析。结果最终纳入59篇文献,共提取针灸处方52条,涉及34个腧穴,总频次为302次。其中使用频率最高的腧穴为天枢(14.90%),其次为足三里(11.59%),第三是中脘(10.26%)。数据分析发现,临床针对IBD治疗多运用募穴、下合穴等特定腧穴,同时主要选穴位于足阳明胃经、任脉等与肠胃关系密切的经脉。关联规则结果表明天枢与足三里配伍使用的关联度最高。结论临床上针灸治疗IBD习惯选用胃经、任脉为主导经脉,募穴、下合穴为主治腧穴,并辅以天枢、足三里等核心配伍,以通过调理脾胃、理气化滞的作用而达到治疗效果。Objective To analyze the rules of acupoint selection in the treatment of inflammatory bowel disease(IBD)in the past 10 years based on data mining method.Methods In this study,relevant clinical research literature on IBD in acupuncture treatment was retrieved in PubMed,Embase,Cochrane Library,Web of Science,SinoMed and CNKI.The selected acupoints in the literature were extracted to form prescriptions and a database of acupoints was established.SPSS 26.0 software was used to analyze the frequency of acupoints,the distribution of meridians and the application of specific acupoints.Results Finally,59 literatures were included,and 52 acupuncture prescriptions were extracted,involving 34 acupoints,with a total frequency of 302.The most frequently used acupoint was Tianshu(14.90%),followed by Zusanli(11.59%)and Zhongwan(10.26%).According to the data analysis,specific acupoints such as Front-mu points and Xiahe points were selected in the clinical treatment of IBD,and the main acupoints were selected in the foot-yangming stomach meridian and the conception vessel.The results of association rules showed that the correlation between Tianshu and Zusanli was the highest.Conclusion In clinical acupuncture treatment of IBD,it is customary to choose stomach meridian and conception vessel as the dominant meridians,and Front-mu points and Xiahe points as the main acupoints,supplemented by Tianshu,Zusanli and other core compatibility,in order to achieve therapeutic effect by regulating the spleen and stomach and regulating qi stagnation.
