摘 要:目的了解高原缺氧环境与血脂和肝功能之间关系。方法选择2022年6月至7月于格尔木某部队医院体检的驻青海格尔木地区(海拔2900 m)和烟台某部队医院体检的驻山东烟台地区(海拔30 m)的官兵5034名,分为平原组(n=1795)和高原组(n=3239),收集基线资料和肝功、血脂的检查结果,采用斯皮尔曼相关性分析和多重线性回归的方法分析其之间相关关系。结果平原组与高原组总胆固醇(TC)、谷草转氨酶与谷丙转氨酶比值(AST/ALT)、高密度脂蛋白胆固醇(HDL-C)、总蛋白、总胆红素、直接胆红素、间接胆红素、血红蛋白、红细胞计数、白蛋白与球蛋白比值、总胆汁酸均有统计学差异,且多重线性回归结果显示年龄、体质量指数(BMI)、高原居住时间、性别、吸烟情况和运动情况可作为甘油三酯(TG)的影响因素,TC的影响因素则为年龄、BMI、高原居住时间、吸烟情况和运动情况。同时TC、TG、HDL-C也是除年龄、BMI、高原居住时间、性别之外的AST/ALT影响因素。结论高原缺氧作为影响因素可以使人体的血脂指标升高,而血脂的变化同时又可以与年龄、BMI等因素共同作用,影响肝脏功能。Objective To investigate the relationship between high-altitude hypoxia environment and blood lipids and liver function.Methods A total of 5034 officers and soldiers stationed in Golmud,Qinghai Province(at an altitude of 2900 m)and Yantai,Shandong Province(at an altitude of 30 m)who underwent physical examination in a military hospital from June to July 2022 were selected and divided into plain group(n=1795)and plateau group(n=3239).The baseline data and the results of liver function and blood lipids were collected,and the correlation between them was analyzed by Spearman correlation analysis and multiple linear regression.Results There were statistical differences in total cholesterol(TC),aspartate aminotransferase/alanine aminotransferase(AST/ALT),high-density lipoprotein cholesterol(HDL-C),total protein,total bilirubin,direct bilirubin,indirect bilirubin,hemoglobin,red blood cell count,albumin/globulin ratio,and total bile acid between plain group and plateau group.The results of multiple linear regression showed that age,body mass index(BMI),living time at high altitude,sex,smoking and exercise could be used as influencing factors of triglyceride(TG),while the influencing factors of TC were age,BMI,living time at high altitude,smoking and exercise.At the same time,TC,TG and HDL-C were also influencing factors of AST/ALT in addition to age,BMI,living time at high altitude and sex.Conclusion Hypoxia at high altitude as an influencing factor can increase the blood lipid index of human body,and the changes of blood lipid can affect liver function together with age,BMI and other factors.
