针刺结合小柴胡汤治疗哮喘急性发作期疗效观察

摘　　要：目的观察针刺结合小柴胡汤治疗支气管哮喘急性发作(AEBA)的临床疗效。方法筛选170例AEBA患者,采用双盲法将其随机分为联合组与对照组各85例。对照组接受西药基础治疗,联合组在对照组基础上采取小柴胡汤与针刺疗法的联合治疗方案,对比两组肺功能[第1秒用力呼气容积(FEV1)、用力肺活量(FVC)与最大呼气流量(PEF)]、免疫功能[免疫球蛋白M(IgM)、免疫球蛋白A(IgA)与免疫球蛋白E(IgE)]、炎性反应[嗜酸粒细胞阳离子蛋白(ECP)、白细胞介素-13(IL-13)与呼出气一氧化氮(FeNO)水平]、中医症状评分[胸闷、咳嗽、喘息以及咯痰]及不良反应(失眠、眩晕等)发生情况。结果治疗后联合组FEV1、FVC与PEF水平明显高于对照组(P<0.05);治疗后联合组IgM、IgA水平明显高于对照组,IgE水平明显低于对照组(P<0.05);联合组ECP、IL-13、FeNO水平明显低于对照组(P<0.05);联合组胸闷、咳嗽、喘息以及咯痰评分明显低于对照组(P<0.05);对照组与联合组不良反应总发生率分别为20.00%与7.06%,联合组明显更低(P<0.05)。结论AEBA患者应用针刺联合小柴胡汤的治疗方案能够改善肺功能与免疫功能,减轻气道炎性反应、临床症状与不良反应。Objective:To analyze the clinical eficacy of a treatment regimen combining acupuncture and Xiaochaihu Decoction for acute exacerbation of bronchial asthma(AEBA).Methods:A total of 170 AEBA patients were screened and randomly divided into a combined group(n=85)and a control group(n=85)using a doubleblind method.The control group received basic Western medicine treatment,while the combined group was additionally treated with Xiaochaihu Decoction and acupuncture.Comparisons were made between the two groups in terms of lung function[forced expiratory volume in one second(FEV1),forced vital capacity(FVC),and peak expiratory flow(PEF)],immune function[immunoglobulin M(IgM),immunoglobulin A(IgA),and immunoglobulin E(IgE)],inflammatory reactions[eosinophil cationic protein(ECP),interleukin-13(IL-13),and fractional exhaled nitric oxide(FeNO)levels],TCM symptom scores[chest tightness,cough,wheezing,and sputum production],and adverse reactions(insomnia,dizziness,etc.).Results:After treatment,the levels of FEV1,FVC,and PEF in the combined group were significantly higher than those in the control group(P<0.05).The levels of IgM and IgA in the combined group were significantly higher than those in the control group,while the level of IgE was significantly lower(P<0.05).The levels of ECP,IL-13,and FeNO in the combined group were significantly lower than those in the control group(P<0.05).The scores for chest tightness,cough,wheezing,and sputum production in the combined group were significantly lower than those in the control group(P<0.05).The total incidence rates of adverse reactions in the control group and the combined group were 20.00%and 7.06%,respectively,with the combined group showing a significantly lower rate(P<0.05).Conclusion:The treatment regimen of acupuncture combined with Xiaochaihu Decoction for AEBA patients can improve lung function and immune function,reduce airway inflammation,alleviate clinical symptoms,and decrease adverse reactions.
