如意金黄膏治疗膝关节积液的早期临床疗效

摘　　要：目的 观察如意金黄膏治疗膝关节积液的早期临床疗效。方法 选择膝关节积液患者共72例作为研究对象,根据患者的首诊日期对其进行分组,奇数日纳入治疗组共33例,偶数日纳入对照组共39例,治疗组给予如意金黄膏外敷治疗,对照组给予双氯芬酸二乙乳胶剂外涂治疗。观察两组患者治疗前、治疗2周后的关节积液量、膝关节肿胀程度和膝关节疼痛视觉模型量表(VAS)评分、C反应蛋白(CRP)、红细胞沉降率(ESR)等指标,进行疗效判定。结果 治疗2周后,两组膝关节VAS评分均较治疗前降低(P <0.01),且治疗组的VAS评分降低更为显著(P <0.05);治疗组的膝关节肿胀程度及关节腔积液积分均较治疗前降低(P <0.01),治疗后的组间差异有统计学意义(P <0.01);治疗后两组的CRP均较治疗前降低(P <0.01),且治疗组的降幅更明显,组间差异有统计学意义(P <0.01);两组ESR均较治疗前降低(P <0.01),但组间差异无统计学意义(P> 0.05)。结论 如意金黄膏可以有效减少膝关节积液,更快速地缓解患者肿痛的临床症状,降低炎性指标表达,提高临床疗效。Objective:To observe the early clinical efficacy of Ruyi Jinhuan Ointment in the treatment of knee joint effusion.Methods:A total of 72 patients with knee joint effusion were selected as study subjects.Patients were grouped based on their first visit date,with those visiting on odd daysassigned to the treatment group(n=33)and those visiting on even days assigned to the control group(n=39).The treatment group received external application of Ruyi Jinhuan Ointment,while the control group was treated with topical diclofenac diethylamine gel.The joint effusion volume,knee swelling degree,knee pain score(VAS),C-reactive protein(CRP),erythrocyte sedimentation rate(ESR),and other indicators were observed before and after 2 weeks of treatment to evaluate the therapeutic effects.Results:After 2 weeks of treatment,the VAS scores for knee pain in both groups decreased compared with before treatment(P<0.01),with a more significant reduction in the treatment group(P<0.05).Knee swelling degree and joint cavity effusion scores in the treatment group also decreased significantly(P<0.01),with statistically significant differences between the two groups after treatment(P<0.01).CRP levels in both groups decreased after treatment(P<0.01),with a more pronounced decrease in the treatment group,showing a significant intergroup difference(P<0.01).Both groups showed reduced ESR levels(P<0.01),but there was no significant difference between the groups(P>0.05).Conclusion:Ruyi Jinhuan Ointment can effectively reduce knee joint effusion,rapidly alleviate clinical symptoms such as swelling and pain,lower inflammatory markers,and improve clinical efficacy.
