柴银亚甲方治疗亚急性甲状腺炎临床观察

摘　　要：目的 观察柴银亚甲方治疗亚急性甲状腺炎的疗效。方法 选取我院亚急性甲状腺炎患者80例作为研究对象,按照随机数表法分为两组各40例,对照组给予常规治疗+布洛芬口服,观察组予常规治疗+柴银亚甲方口服。疗程6周。对比两组患者临床疗效、血常规、甲状腺功能、炎症因子水平、免疫功能、不良反应。结果 观察组患者总有效率95.00%,高于对照组的77.50%(P <0.05);治疗后,观察组中性粒细胞(NEUT)、中性粒细胞与淋巴细胞比值(NLR)较对照组低(P <0.05);游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)较对照组低,促甲状腺激素(TSH)较对照组高(P <0.05);白细胞介素-3(IL-3)、白细胞介素-2(IL-2)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、红细胞沉降率(ESR)、C反应蛋白(CRP)水平较对照组降低(P <0.05);补体C3、补体C4、免疫球蛋白A(IgA)、免疫球蛋白M(IgM)、免疫球蛋白G(IgG)水平较对照组降低,CD4+CD25+CD127-Treg/CD4+、单个核细胞Foxp3阳性率较对照组升高(P <0.05);两组患者不良反应差异无统计学意义(P> 0.05)。结论 柴银亚甲方治疗亚急性甲状腺炎可有效提高临床疗效,改善甲状腺功能,缓解炎症水平,调节免疫功能且不会增加不良反应,安全性较高。Objective:To observe the efficacy of Chaiyin Yajia Decoction in the treatment of subacute thyroiditis.Methods:80 patients with subacute thyroiditis treated at our hospital were selected as study subjects and randomly divided into two groups,40 each using a random number table method.The control group received conventional treatment plus ibuprofen,while the observation group received conventional treatment and oral Chaiyin Yajia Decoction.The course of treatment lasted for 6 weeks.Clinical efficacy,blood routine tests,thyroid function,inflammatory factor levels,immune function,and adverse reactions were compared between the two groups.Results:The total effective rate in the observation group was 95.00%,which was significantly higher than 77.50%in the control group(P<0.05).After treatment,neutrophil(NEUT)and neutrophil-to-lymphocyte ratio(NLR)in the observation group were lower than those in the control group(P<0.05);free triodothyronine(FT3)and free thyroxine(FT4)were lower,while thyroid-stimulating hormone(TSH)was higher in the observation group compared with the control group(P<0.05);interleukin-3(IL-3),interleukin-2(IL-2),interleukin-6(IL-6),tumor necrosis factor-alpha(TNF-α),erythrocyte sedimentation rate(ESR),and C-reactive protein(CRP)levels were lower in the observation group(P<0.05);complement C3,complement C4,immunoglobulin A(IgA),immunoglobulin M(IgM),and immunoglobulin G(IgG)levels were lower in the observation group,while CD4+CD25+CD127-Treg/CD4+,and the positivity rate of mononuclear cell Foxp3 were higher in the observation group compared with the control group(P<0.05).There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion:Chaiyin Yajia Decoction can effectively improve clinical efficacy,enhance thyroid function,alleviate inflammation,regulate immune function,and does not increase adverse reactions,indicating high safety in treating subacute thyroiditis.
