摘 要:目的:分析老年膝关节骨性关节炎(KOA)患者内外髁软骨中白细胞介素-24(IL-24)与白细胞介素-1(IL-1)的表达情况。方法:收集2022年3月至2023年6月行全膝关节置换术且符合纳入标准的30例老年KOA患者,无菌条件下取股骨内侧髁磨损区软骨粒2~5 g为对照组(X-N),取股骨外侧髁未磨损区软骨粒2~5 g为试验组(X-A)。两组均以RT-PCR检测软骨组织中IL-24、IL-1两种细胞因子的表达。结果:IL-24的2^(-ΔΔCt)在对照组中为(6.57±4.23),在试验组中为(0.54±0.47);IL-1的2^(-ΔΔCt)在对照组中为(2.70±3.16),在试验组中为(0.23±0.34);IL-24和IL-1在两组软骨组织中的2^(-ΔΔCt)比较,差异有统计学意义(P<0.05)。结论:IL-24与IL-1在老年KOA患者内外髁软骨中的表达存在明显差异,IL-24的高表达以拮抗KOA发生过程中软骨分解代谢过程,因此临床可早期进行炎性细胞因子水平干预,以减少或延缓KOA的发生发展。Objective:To analyze the expression of interleukin-24(IL-24)and interleukin-1(IL-1)in the medial and lateral femoral condylar cartilage of elderly patients with knee osteoarthritis(KOA).Methods:Cartilage samples were collected from 30 elderly KOA patients who underwent total knee arthroplasty(TKA)between March 2022 and June 2023 and met the inclusion criteria.Under sterile conditions,2~5 g of cartilage particles from the medial femoral condyle worn area were designated as the control group(X-N),while 2~5 g of cartilage particles from the lateral femoral condyle nonworn area were designated as the experimental group(X-A).RT-PCR was used to detect the expression of IL-24 and IL-1 in both groups.Results:The 2^(-ΔΔCt) values of IL-24 were 6.57±4.23 in the control group and 0.54±0.47 in the experimental group.For IL-1,the 2^(-ΔΔCt) values were 2.70±3.16 in the control group and 0.23±0.34 in the experimental group.The differences in 2^(-ΔΔCt) values of IL-24 and IL-1 between the two groups were statistically significant(P<0.05).Conclusion:IL-24 and IL-1 exhibit distinct expression patterns in the medial and lateral femoral condylar cartilage of elderly KOA patients.The high expression of IL-24 may antagonize cartilage catabolism during KOA progression,suggesting that early intervention targeting inflammatory cytokine levels could reduce or delay the onset and progression of KOA.
