摘　　要：目的:分析心脏超声参数在评估心房颤动导管射频消融术(RFCA)术后复发中的应用价值。方法:选取2022年2月至2024年2月在新乡市第一人民医院接受RFCA治疗的心房颤动患者180例,依据术后随访1年内有无术后复发将其分为未复发组和复发组。对比两组患者的一般资料及心脏超声参数,以Logistic回归分析RFCA患者术后复发的影响因素,并绘制ROC曲线分析心脏超声参数在RFCA术后复发中的诊断效能。结果:随访期间,72例患者出现术后复发,复发率为40%。复发组患者左房总排空分数(LATEF)、左房主动排空分数(LAaEF)、二尖瓣瓣环舒张晚期峰值速度(A')、二尖瓣瓣环舒张早期峰值速度(E')、二尖瓣瓣环收缩期收缩峰值速度(s')等心脏超声相关参数均低于未复发组(P<0.05)。LATEF、LAaEF、A'、E'、s'均为心房颤动患者RFCA术后复发的影响因素(P<0.05),上述指标诊断心房颤动患者RFCA术后复发的AUC分别为0.878、0.886、0.875、0.834、0.813,均有良好的诊断预测价值(P<0.05)。结论:心脏超声参数LATEF、LAaEF、A'、E'、s'较低均为心房颤动患者RFCA术后复发的影响因素,且上述参数均在心房颤动RFCA术后复发评估中具有较高的应用价值。Objective:To analyze the value of cardiac ultrasound parameters in evaluating recurrence after catheter radiofrequency ablation(RFCA)for atrial fibrillation.Methods:A total of 180 atrial fibrillation patients who underwent RFCA at Xinxiang First People’s Hospital from February 2022 to February 2024 were enrolled.Based on recurrence within 1 year postoperatively,they were divided into a non-recurrence group and a recurrence group.General data and cardiac ultrasound parameters were compared between the two groups.Logistic regression was used to analyze influencing factors for postoperative recurrence in RFCA patients,and ROC curves were plotted to evaluate the diagnostic efficacy of cardiac ultrasound parameters in predicting recurrence.Results:During follow-up,72 patients experienced recurrence,with a recurrence rate of 40%.The recurrence group exhibited significantly lower cardiac ultrasound parameters compared to the non-recurrence group,including left atrial total emptying fraction(LATEF),left atrial active emptying fraction(LAaEF),late diastolic peak velocity of the mitral annulus(A'),early diastolic peak velocity of the mitral annulus(E'),and systolic peak velocity of the mitral annulus(s')(all P<0.05).Logistic regression identified LATEF,LAaEF,A',E',and s'as independent factors influencing postoperative recurrence(all P<0.05).The AUC values of these parameters for diagnosing recurrence were 0.878,0.886,0.875,0.834,and 0.813,respectively,demonstrating strong predictive value(all P<0.05).Conclusion:Cardiac ultrasound parameters LATEF,LAaEF,A',E',and s'are significant factors influencing recurrence after RFCA for atrial fibrillation,and these parameters hold high clinical value in postoperative recurrence assessment.
