摘 要:目的探讨磁共振T1-mapping技术定量评价2型糖尿病(T2DM)患者肾功能的价值。方法收集36例T2DM患者资料,依据尿白蛋白/肌酐比值分为单纯T2DM组(A组,18例)和T2DM合并糖尿病肾脏病组(B组,18例)。分析两组相关资料,比较磁共振T1-mapping扫描测量两侧肾皮质T1值。结果B组两侧肾皮质T1值均高于A组同侧肾皮质T1值(P<0.05)。B组两侧肾皮质T1值均与估算的肾小球滤过率呈负相关(P<0.05),左侧肾皮质T1值与BMI呈负相关(P<0.05),右侧肾皮质T1值与胱抑素C、血清肌酐和尿β2微球蛋白呈正相关(P<0.05)。结论磁共振T1-mapping技术对T2DM患者肾脏定量评价具有可行性,T1值可间接预测单侧肾功能变化。Objective To explore the value of quantitative evaluation of renal function using magnetic resonance Tl-mapping technology in the patients with type 2 diabetes(T2DM).Methods A total of 36 patients with T2DM were collected and divided into two groups of A(diagnosed as simple T2DM,18 cases)and B(diagnosed as T2DM and diabetic kidney disease,18 cases)based on the urinary albumin/creatinine ratio.The relevant data of the two groups were analyzed.The patients of two groups underwent magnetic resonance T1-mapping scans of the kidneys and the T1 value of the renal cortex was compared.Results The T1 values of both sides of the renal cortex in group B were higher than those in the ipsilateral renal cortex in group A(P<0.05).The T1 values of both sides of the renal cortex in group B were negatively correlated with eGFR(P<0.05),while which of the left side of the renal cortex was negatively correlated with BMI(P<0.05).The T1 value of the right side of the renal cortex in group B was positively correlated with CysC,SCr andβ_2-MG(P<0.05).Conclusion Magnetic resonance T1-mapping technology is feasible for quantitative evaluation of kidneys in T2DM patients,and T1 values can indirectly predict unilateral renal function changes.
