摘　　要：目的 探讨复合环泊酚时雷米芬太尼用于无痛人工流产术麻醉时的半数有效剂量(ED_(50))。方法 选取择期行无痛人工流产术患者25例,麻醉诱导采用环泊酚0.4mg/kg和雷米芬太尼。根据Dixon序贯法确定雷米芬太尼剂量:若前一例患者置入窥器或术中出现体动、皱眉等阳性反应,下一例雷米芬太尼增加0.1μg/kg;若置入窥器或术中反应为阴性,下一例雷米芬太尼减少0.1μg/kg。至阳性转为阴性形成7个拐点结束试验。采用Probit法计算复合环泊酚时雷米芬太尼用于无痛人工流产术的ED_(50)、95%有效剂量(ED_(95))及相应的95%置信区间(95%CI)。结果 复合环泊酚时,雷米芬太尼用于无痛人工流产术的ED_(50)(95%CI)为1.109(0.951~1.327)μg/kg,ED_(95)(95%CI)为1.362(1.225~3.639)μg/kg。所有患者未出现注射痛、头痛和呼吸抑制等不良反应。结论 复合环泊酚用于无痛人工流产术麻醉时,雷米芬太尼的ED_(50)为1.109μg/kg。Objective To determine the median effective dose(ED_(50)) of remifentanil when it is combined with ciprofol for painless abortion.Methods A total of 25 patients undergoing painless abortion were anesthetized with cypofol 0.4mg/kg and remifentanil.The dose of remifentanil was determined according to Dixon's up-and-down sequential method.If the previous patient had positive reaction such as body movement or frowning during the placement of a speculum or surgery,the remifentanil of the next patient increased by 0.1μg/kg.If the reaction was negative,the remifentanil of the next patient decreased by 0.1μg/kg.The experiment ended with 7 inflection points formed when positive turned negative.The Probit method was used to calculate the ED_(50),95% effective dose(ED_(95)),and corresponding 95% confidence interval(95%CI) of remifentanil combined with ciprofol for painless abortion.Results The ED_(50 )(95%CI) of remifentanil was 1.109(0.951-1.327) μg/kg,and the ED_(95)(95%CI) was 1.362(1.225-3.639) μg/kg.when it was combined with ciprofol for painless abortion.No adverse reactions such as injection pain,headache and respiratory depression occurred in all the patients.Conclusion The ED_(50) of remifentanil is 1.109μg/kg when it is combined with ciprofol for painless abortion.
