肌骨超声引导下体外冲击波疗法联合正骨手法对青少年特发性脊柱侧弯的疗效

摘　　要：目的探讨肌骨超声引导下体外冲击波疗法(ESWT)联合正骨手法对青少年特发性脊柱侧弯的临床疗效及安全性。方法96例青少年特发性脊柱侧弯患者分为两组。对照组(44例)采用肌骨超声引导下ESWT治疗;研究组(52例)在对照组基础上联合正骨手法治疗。每周治疗1次,疗程3个月。比较两组疗效,记录相关并发症发生情况。结果与治疗前相比,治疗后两组患者的Cobb角、躯干旋转角度、顶椎偏移距离、喙突高度差、锁骨肋骨交点高度差、锁骨角、T_(1)倾斜角和数字评定量表评分均减小(P<0.01),且研究组小于对照组(P<0.01)。研究组Cobb角矫正率高于对照组(P<0.01)。两组患者治疗期间均未出现严重并发症。结论与肌骨超声引导下ESWT治疗比较,联合应用正骨手法治疗青少年特发性脊柱侧弯有助于减小患者的Cobb角、躯干旋转角度及顶椎偏移距离,促进肩平衡的恢复,且不增加不良反应。Objective To explore the clinical efficacy and safety of muscle bone ultrasound-guided extracorporeal shock wave therapy(ESWT)combined with orthopedic technique in the treatment of adolescent idiopathic scoliosis.Methods Ninety-six patients with adolescent idiopathic scoliosis were divided into two groups.Group C(44 cases)received muscle bone ultrasound-guided ESWT,and group A(52 cases)received additional orthopedic technique on the basis of group C.The treatments were performed once a week for 3 months.The efficacy of the treatments was compared between the two groups.The treatment-related complications were recorded.Results Compared to before treatment,the Cobb angle,trunk rotation angle,vertebral displacement distance,coracoid process height difference,clavicle rib intersection height difference,clavicle angle,T_(1)tilt angle,and numerical rating scale score in both groups were decreased after treatment(P<0.01),which in group A were smaller than those in group C(P<0.01).The Cobb angle correction rate in group A was higher than that in group C(P<0.01).No patient experienced serious complications during the treatment period in both groups.Conclusion Compared to muscle bone ultrasound-guided ESWT,combined use of orthopedic technique in the treatment of adolescent idiopathic scoliosis can reduce the Cobb angle,trunk rotation angle and vertex displacement distance and promote the recovery of shoulder balance without increasing the treatment-related complications.
