摘 要:目的 探讨真菌免疫荧光染色对皮肤浅部真菌感染的快速诊断价值。方法 对48例初诊为浅部真菌感染的患者皮损刮取鳞屑,分别进行真菌免疫荧光染色、真菌涂片及真菌培养。以真菌培养结果为金标准,计算真菌免疫荧光染色和真菌涂片两种检查方法的灵敏度、特异度和一致率,并记录真菌免疫荧光染色及真菌涂片的镜检时间。结果 真菌免疫荧光染色诊断皮肤浅部真菌感染的灵敏度和一致率高于真菌涂片(93.8%vs.78.1%和85.4%vs.72.9%)(P<0.05),但两种检查方法的特异度比较无统计学差异(68.8%vs.62.5%)(P>0.05)。真菌免疫荧光染色的镜检时间短于真菌涂片[(28.2±9.7)s vs.(96.3±22.4)s](P<0.05)。结论 相较于真菌涂片,真菌免疫荧光染色诊断皮肤浅部真菌感染的灵敏度和一致率更高,镜检时间更短。Objective To investigate the value of fungal immunofluorescence staining in the rapid diagnosis of superficial fungal infections of the skin.Methods Scales from skin lesions of 48 patients initially diagnosed with superficial fungal infections were collected for fungal immunofluorescence staining,fungal smear and fungal culture.Using the result of fungal culture as the gold standard,the sensitivity,specificity and agreement rate of fungal immunofluorescence staining and fungal smear were calculated.The microscopic examination time for fungal immunofluorescence staining and fungal smear was also recorded.Results The sensitivity and agreement rate of fungal immunofluorescence staining were higher than those of fungal smear(93.8% vs.78.1% and 85.4% vs.72.9%)(P<0.05).There was no statistical difference in the specificity between the two methods(68.8% vs.62.5%)(P>0.05).The microscopic examination time for fungal immunofluorescence staining was shorter than that for fungal smear [(28.2±9.7) seconds vs.(96.3±22.4) seconds](P<0.05).Conclusion Compared to fungal smear,fungal immunofluorescence staining has higher sensitivity and agreement rate in diagnosing superficial fungal infections of the skin,with a shorter microscopic examination time.
