可弯曲负压吸引鞘在大于2 cm上尿路结石输尿管软镜碎石术中的应用效果研究

摘　　要：目的研究可弯曲负压吸引鞘在大于2 cm上尿路结石输尿管软镜碎石术(FURL)术中的应用效果。方法选择本院2022年9月至2024年6月收治的102例上尿路结石患者作为研究对象。按照随机数表法分为观察组(51例,采用可弯曲负压吸引鞘联合FURL治疗)及对照组(51例,采用常规输尿管导入鞘联合FURL治疗)。比较两组手术指标(手术时间、术中出血量、术后结石清除率、住院时间),肾功能(胱抑素C、血肌酐及尿素氮)及并发症发生率。结果观察组手术时间长于对照组(P<0.05),观察组术后3 d结石清除率高于对照组(P<0.05),两组术中出血量、住院时间相比无差异(P>0.05)。观察组术后肾功能指标低于对照组(P<0.05)。观察组并发症发生率低于对照组(P<0.05)。结论可弯曲负压吸引鞘能够提高大于2 cm上尿路结石患者的结石清除率,对肾功能的影响更小,可减少术后并发症。Objective To investigate the effect of flexible negative pressure suction sheath in ureteral flexible lithotripsy(FURL)for upper urinary calculi larger than 2 cm.Methods 102 upper urinary calculi patients admitted to our hospital from September 2022 to June 2024 were divided into observation group(51 cases with flexible negative pressure aspiration sheath combined with FURL)and control group(51 cases with conventional ureteral insertion sheath combined with FURL)according to random number table method.The operation indexes(operation time,intraoperative blood loss,postoperative stone clearance rate and hospitalization time),renal function(cystatin C,serum creatinine and urea nitrogen)and the incidence of complications were compared between the two groups.Results The operation time in the observation group was longer than that in the control group(P<0.05),and the stone clearance rate in the observation group was higher than that in the control group at the 3rd day after operation(P<0.05),there was no difference between the two groups in intraoperative blood loss and hospitalization time(P>0.05).The postoperative renal function in the observation group was lower than that in the control group(P<0.05).The complication rate in the observation group was lower than that in the control group(P<0.05).Conclusion Bendable negative pressure suction sheath can improve the stone clearance rate of patients with upper urinary tract stones larger than 2 cm,have less impact on renal function,and reduce postoperative complications.
