黄芪桂枝五物汤防治奥沙利铂致周围神经毒性的系统评价和Meta分析

摘　　要：目的系统评价黄芪桂枝五物汤对奥沙利铂致周围神经毒性(OIPN)的防治作用,为临床提供循证参考依据。方法计算机检索PubMed、EMbase、Cohrance、中国知网(CNKI)、维普(VIP)以及万方(Wanfang)数据库,检索时间从2012年1月~2022年12月,全面纳入符合标准的随机对照研究,根据Cochrane风险偏倚评估工具对纳入的文献进行质量评价,并采用RevMan 5.4软件进行分析。结果共纳入15个随机对照研究,共计835例患者。Meta分析结果表明,黄芪桂枝五物汤在影响OIPN总发生率(RR=0.51,95%Cl:0.44~0.60,Z=8.57,P<0.00001)、严重OIPN总发生率(RR=0.44,95%Cl:0.29~0.67,Z=3.78,P=0.0002)方面,均优于空白对照组或甲钴胺组,而对比四君子汤差异无统计学意义。结论黄芪桂枝五物汤在奥沙利铂致OIPN的临床防治中具有显著优势,且安全性强,可以为OIPN的临床决策提供参考。Objective To systematically evaluate the preventive and therapeutic effects of Huangqi Guizhi Wuwu Decoction on oxaliplatin-induced peripheral neurotoxicity(OIPN)induced by oxaliplatin.Methods A comprehensive computerized search was conducted across multiple databases,including PubMed,EMbase,the Cochrane Library,China National Knowledge Infrastructure(CNKI),VIP Database,and Wanfang Data,covering studies published from January 2012 to December 2022.All randomized controlled trials(RCTs)meeting the predefined eligibility criteria were systematically included.The methodological quality of the selected studies was rigorously evaluated using the Cochrane Risk of Bias Tool.Statistical analyses were subsequently performed with RevMan 5.4 software to synthesize the evidence.Results A total of 15 randomised controlled studies with a total of 835 patients were included.Meta-analysis results showed that Huangqi Guizhi Wuwu Decoction,in terms of affecting the total incidence of OIPN(RR=0.51,95%Cl:0.44~0.60,Z=8.57,P<0.00001),the total incidence of severe OIPN(RR=0.44,95%Cl:0.29~0.67,Z=3.78,P=0.0002),were all superior to the blank control group or methylcobalamin group,while the difference was not statistically significant when comparing with Sijunzi Decoction.Conclusion Huangqi Guizhi Wuwu Decoction has significant advantages in the clinical management of OIPN with a strong safety profile,which can be used to inform clinical decision-making in OIPN.
