从“正虚邪滞”理论探讨线粒体自噬在糖尿病溃疡的调控机制

摘　　要：线粒体自噬可靶向清除功能异常的线粒体,促进线粒体更新与再循环,维持细胞稳态。糖尿病溃疡(DU)病程漫长,发病机制复杂。研究表明,线粒体自噬是DU发生发展的关键病理机制。“正虚邪滞”是DU的核心病机。“正虚”指胰脾亏损,为发病之本;“邪滞”指胰脾亏损日久,致湿热瘀等邪气阻滞,为发病之标。线粒体自噬不足或亢进,则降解能力减弱,致疾病的发生,与“正虚邪滞”病机具有相通之处。因此,将DU“正虚邪滞”理论与线粒体自噬结合,探讨二者在DU中的关联性,旨在阐释“正虚邪滞”病机的科学内涵,总结中医药影响线粒体自噬介入DU的治疗策略,期望为DU的防治、基础研究提供参考。Mitophagy can target and eliminate dysfunctional mitochondria,promote mitochondrial renewal and recycling,and maintain cellular homeostasis.Diabetic ulcer(DU)has a long disease course and a complex pathogenesis.Studies have shown that mitophagy is a key pathological mechanism in the occurrence and development of DU."Deficiency of healthy qi and stagnation of pathogenic factors"is the core pathogenesis of DU."Deficiency of healthy qi"refers to the deficiency of the pancreas and spleen,which is the root of the disease;"Stagnation of pathogenic factors"means that the long-term deficiency of the pancreas and spleen leads to the stagnation of pathogenic factors such as dampness-heat and stasis,which is the manifestation of the disease.Insufficient or excessive mitophagy leads to weakened degradation ability and the occurrence of diseases,which has similarities with the pathogenesis of"deficiency of healthy qi and stagnation of pathogenic factors".Therefore,by combining the theory of"deficiency of healthy qi and stagnation of pathogenic factors"of DU with mitophagy and exploring the correlation between the two in DU,the aim is to explain the scientific connotation of the pathogenesis of"deficiency of healthy qi and stagnation of pathogenic factors",summarize the treatment strategy of traditional Chinese medicine affecting mitophagy in the intervention of DU,and hope to provide a reference for the prevention,treatment and basic research of DU.
